Introduction

A subtype of MICROSCOPIC COLITIS, characterized by chronic watery DIARRHEA of unknown origin, a normal COLONOSCOPY but abnormal histopathology on BIOPSY. Microscopic examination of biopsy samples taken from the COLON show larger-than-normal band of subepithelial COLLAGEN.

Rifaximin-treatment of Collagenous Colitis:

The hypothesis of this study is that an altered gut microbiota is a contributory factor in initiating an inflammatory process in the colonic mucosa leading to collagenous colitis. The investigators suggest that treatment with budesonide reduces the inflammation without treating the underlying cause. In this trial the investigators will try to modullate gut microbiota by adding rifaximin. The aim of this study is to assess if 4 weeks treatment with rifaximin as a supplement to a standard course of budesonide against active CC can reduce the risk of relapse after treatment cessation.

Cholesterol crystals in the pathogenesis of atherosclerosis

The presence of cholesterol crystals (CCs) in tissues was first described more than 100 years ago. CCs have a pathogenic role in various cardiovascular diseases, including myocardial infarction, aortic aneurysm and, most prominently, atherosclerosis. Although the underlying mechanisms and signalling pathways involved in CC formation are incompletely understood, numerous studies have highlighted the existence of CCs at various stages of atheroma progression. In this Review, we summarize the mechanisms underlying CC formation and the role of CCs in cardiovascular disease. In particular, we explore the established links between lipid metabolism across various cell types and the formation of CCs, with a focus on CC occurrence in the vasculature. We also discuss CC-induced inflammation as one of the pathogenic features of CCs in the atheroma. Finally, we summarize the therapeutic strategies aimed at reducing CC-mediated atherosclerotic burden, including approaches to inhibit CC formation in the vasculature or to mitigate the inflammatory response triggered by CCs. Addressing CC formation might emerge as a crucial component in our broader efforts to combat cardiovascular disease.

Cholecystectomy is a Risk Factor for Microscopic Colitis: A Nationwide Population-based Matched Case Control Study

BACKGROUND & AIMSStudies have linked bile acid malabsorption and microscopic colitis (MC), with some patients with MC responding to treatment with bile acid sequestrants. However, the literature on cholecystectomy as a risk factor for MC is inconclusive. Therefore, we investigated the relationship between cholecystectomy and MC on a nationwide scale to provide more definitive insights.METHODSWe conducted a nationwide matched case-control study involving 13,554 patients diagnosed with MC between 1981 and 2017 in Sweden who were matched to 64,886 controls. Data on MC were obtained from Swedish pathology registers, and controls were randomly selected from the general population and matched according to birth year, sex, county of residence, and calendar year. Moreover, MC-free full siblings to patients with MC were identified. Information on cholecystectomy was collected from the Swedish National Patient Register. Adjusted odds ratios (aORs) were calculated using multivariable-adjusted conditional logistic regression.RESULTSThe median age at diagnosis was 63.5 years (interquartile range [IQR], 51.0-73.4 years), and 72.3% of MC patients were women. Among patients with MC, 342 (2.5%) had undergone a cholecystectomy before diagnosis, compared with 687 (1.1%) in the control group. This yielded an aOR of 2.36 (95% confidence interval [CI], 2.07-2.69) for earlier cholecystectomy in patients with MC. The corresponding aORs for collagenous colitis and lymphocytic colitis were 1.87 (95% CI, 1.48-2.36) and 2.65 (95% CI, 2.26-3.12), respectively. When compared with siblings, the aOR was 1.49 (95% CI, 1.21-1.85).CONCLUSIONSCholecystectomy is associated with an increased risk of subsequent MC. These findings have implications for surgeons and general practitioners and underscore the need for further research into the underlying association between bile acid and MC.

Fish Tank-AssociatedMycobacterium MarinumInfection in an Immunocompromised Host

Mycobacterium marinuminfection can be challenging to diagnose and treat, especially in immunocompromised patients on tumour necrosis factor alpha inhibitors. A patient with collagenous colitis on infliximab presented with ulcerated plaques accompanied by nodules in a sporotrichoid fashion. Initial treatments failed until a biopsy confirmedM. marinum. Multiple antibiotic regimens, including rifampicin and ciprofloxacin, were required due to resistance. The case underscores the importance of patient education on avoiding aquatic environments, considering atypical mycobacterial infections in differentials for immunocompromised patients with sporotrichoid lesions, and prioritising urgent diagnostic biopsies. Treatment can be lengthy, often requiring up to 12 months, emphasising the need for ongoing patient counselling and multidisciplinary management. Current guidelines recommend a combination of ethambutol and macrolides, though further research is necessary to optimise treatment protocols.
