A Randomized, Placebo-controlled, Double-blind, Multi-center Trial to Assess Efficacy and Safety of Octreotide Subcutaneous Depot (CAM2029) in Patients With Symptomatic Polycystic Liver Disease

The purpose of the trial is to compare the effectiveness and safety of 2 treatment regimens of CAM2029 (given weekly or every 2 weeks) to placebo in participants with symptomatic PLD, either isolated as in autosomal dominant PLD (ADPLD) or associated with autosomal dominant polycystic kidney disease (ADPKD).
In the Treatment Period of the trial, participants will be allocated at random to 1 of the 3 treatment arms in a 1:1:1 ratio. After completing the Treatment Period (53 weeks) participants may proceed to a 120-week open-label extension part of the trial and then only receive the same CAM2029 treatment.
The active ingredient in CAM2029, octreotide, is administered as a subcutaneous depot using Camurus' FluidCrystal® technology.

Abstract:

Acromegaly is a chronic multisystem disorder in which biochemical control remains suboptimal for many patients. Even when control is achieved, treatment burden is often high despite surgery and a spectrum of established safe and effective therapies. In this narrative review of multinational late-stage clinical trials through October 2025, we summarize drug developments that may broaden individualized medical strategies. Oral somatostatin receptor ligands are now available in the United States, underscoring a shift toward more convenient long-term management. In parallel, non-oral innovations continue to advance. Octreotide subcutaneous depot (CAM2029) maintains biochemical control with acceptable safety and may reduce treatment burden. Emerging pituitary-directed therapy with longer application intervals (Debio 4126) also shows pharmacologic activity consistent with effective disease control. Growth hormone (GH) receptor–directed agents (ALXN2420, MAR002) likewise demonstrate activity aligned with considerable biochemical and clinical benefit. Collectively, these agents may address unmet needs by broadening the scope of options to control production of hepatic insulin-like growth factor I (IGF-I) and to attenuate additional extrahepatic GH signaling. They may also help ease the practical burden of chronic therapy.

Context:

Acromegaly, characterized by excessive GH and insulin-like growth factor-1 (IGF-1), impacts quality of life (QoL) and mortality. Standard of care (SoC; octreotide long-acting repeatable or lanreotide autogel) treatment typically requires healthcare provider administration. CAM2029, a novel subcutaneous octreotide depot with increased bioavailability using FluidCrystal technology, enables self-administration and room-temperature storage.

Objective:

Assess superiority of CAM2029 vs placebo for biochemical control in patients with controlled acromegaly.

Design:

24-week, multinational, randomized, double-blind, phase 3 trial (NCT04076462).

Setting:

45 sites; 10 countries.

Patients:

72 patients on SoC with biochemical control at screening [IGF-1 ≤upper limit of normal (ULN); mean GH <2.5 μg/L].

Interventions:

Patients were randomized 2:1 to once-monthly CAM2029 (n = 48) or placebo (n = 24).

Main Outcome Measures:

The primary endpoint was proportion of patients with IGF-1 ≤ULN (week 22/24 mean), with dose-reduced patients classified as nonresponders; first key secondary endpoint was the same, including dose-reduced responders. The second key secondary endpoint was proportion of patients with IGF-1 ≤ULN (week 22/24) and mean GH <2.5 μg/L (week 24).

Results:

At week 22/24 (intention-to-treat analysis), CAM2029-treated patients demonstrated superior response rates vs placebo for IGF-1 (72.2% vs 37.5%; risk difference: 34.6, 95% confidence interval: 11.3, 57.9; P = .0018) and combined IGF-1/GH (70.0% vs 37.5%; P = .0035). CAM2029-treated patients had well-controlled symptoms, improved QoL, and treatment satisfaction vs placebo and baseline. CAM2029 was well tolerated; safety was consistent with SoC.

Conclusion:

CAM2029 provides a convenient and effective treatment option for acromegaly, with superior biochemical control vs placebo. Symptom control, QoL, and satisfaction were improved from baseline SoC.

The currently available somatostatin receptor ligands (SRLs) and growth hormone (GH) antagonists are used to control levels of GH and insulin-like growth factor 1 (IGF-1) in patients with acromegaly. However, these therapies are limited by wide variations in efficacy, associated adverse effects and the need for frequent injections. A phase III trial of oral octreotide capsules demonstrated that this treatment can safely sustain suppressed levels of GH and IGF-1 and reduce the severity of symptoms in patients with acromegaly previously controlled by injectable SRL therapy, with the added benefit of no injection-site reactions. Phase I and phase II trials of the pan-selective SRL DG3173, the liquid crystal octreotide depot CAM2029 and an antisense oligonucleotide directed against the GH receptor have shown that these agents can be used to achieve biochemical suppression in acromegaly and have favourable safety profiles. This Review outlines the need for new therapeutic agents for patients with acromegaly, reviews clinical trial data of investigational agents and considers how these therapies might best be integrated into clinical practice.

Camurus receives second manufacturing-linked CRL from FDA for Oclaiz acromegaly drug
