Osteoarthritis is a degenerative, chronic, and progressive joint disease considered as the most common joint disorder worldwide. In healthy joints, there is a continuous process of natural breakdown and repair of cartilage. This process becomes disrupted in OA, leading to degeneration and loss of articular cartilage, along with other joint changes, including subchondral bone remodeling, osteophyte formation, thickening of the capsule and sometimes inflammation of the synovia. JTA-004 is a plasma protein solution supplemented with HA and clonidine developed as a single IA injection for the treatment of knee OA. Local administration of JTA-004 into the joint cavity is intended to relieve chronic pain in subjects suffering from knee OA and discomfort associated with the IA administration.
The JTA-004 Phase III study is a placebo and active-controlled, randomized, double-blind study to evaluate the potential of a single, intra-articular injection of JTA-004 to reduce osteoarthritic pain in the knee at 3 months compared to placebo or active comparator. The study is expected to enrol 742 patients with mild to moderate symptomatic knee osteoarthritis in 22 centres in 6 European countries and Hong Kong SAR.

Osteoarthritis (OA) is the most common joint disease affecting millions of people around the world, for which there is unfortunately no cure. Among existing therapies, viscosupplementation, i.e., the injection of hyaluronic acid into the joint, has an established place in the symptomatic treatment of knee OA.
The present Phase IIb/III aiming to assess the safety and efficacy of JTA-004 is organized in two phases. With results obtained in the first phase the best dose of JTA-004 is determined, and the efficacy of the selected dose will then be confirmed in the second phase.

OBJECTIVES:
Intra-articular treatments for osteoarthritis (OA) are limited by their efficacy, safety, or duration of response. JTA-004 is a potential novel treatment for OA for intra-articular (IA) injection, combining hyaluronic acid (HA) and clonidine with human plasma to enhance the effects of HA. The objectives of the trial were to evaluate the efficacy and safety of JTA-004 in participants with symptomatic knee OA, where the primary objective was evaluating the efficacy of JTA-004 in terms of WOMAC pain, compared to placebo.
DESIGN:
The trial was a multicenter, Phase 3, randomized, double-blind, placebo- and active-controlled clinical trial, evaluating the efficacy and safety of a single IA injection of JTA-004, compared to saline (primary hypothesis) and an HA-comparator (Synvisc-One®) (secondary hypothesis) in knee OA with a Kellgren-Lawrence grade of 2 or 3. For the secondary hypothesis, non-inferiority of JTA-004 to active comparator by comparing the 2 treatment groups on the mean differences in WOMAC pain with a non-inferiority margin of Δ = 10 mm. Primary efficacy endpoint was the change from baseline in WOMAC pain to Month 3. The main secondary efficacy endpoints included changes from baseline in WOMAC function and stiffness at Months 3 and 6, OMERACT-OARSI responder rates, global assessments, and use of rescue medication. Safety assessments were based on adverse events (AE) reporting, and post-injection vital signs.
RESULTS:
A total of 746 participants were randomized, of which 687 (92.1%) completed the trial. The results indicated no significant differences in the primary endpoint between JTA-004 and placebo (LSmean difference: -1.50 mm, 95%CI 5.12; 2.12, p= 0.42) or Synvisc-One® (LSmean difference: 2.40, 95% CI: -1.22; 6.02, p=0.20) nor in either of the efficacy outcomes of the main study population. The safety and tolerability of JTA-004 was good, and there were no differences in the frequency of any of the reported AEs or trial discontinuations between the study groups.
CONCLUSIONS:
A single IA injection of JTA-004 was not superior to a saline solution (LSmean difference: -1.50 mm, 95%CI 5.12; 2.12, p= 0.42) or Synvisc-One® (LSmean difference: 2.40, 95% CI: -1.22; 6.02, p=0.20) for the treatment of OA symptoms in the overall study population.

Abstract:
Background:
New minimally invasive treatments are vital to delay joint replacement surgery in patients with knee osteoarthritis. This study was designed to select the most effective among three formulations of an enhanced protein solution containing clonidine, hyaluronic acid, and human plasma (JTA-004), and compare the safety and efficacy of intra-articular administration of the selected formulation with a reference treatment (hyaluronic acid) in symptomatic knee osteoarthritis patients.
Methods:
In this two-stage, double-blind, phase II/III study conducted in 12 Belgian centers, 50–79-year-old patients with primary knee osteoarthritis were randomized (1:1:1:1) to receive one dose of one of three JTA-004 formulations (differing in clonidine concentration [50 or 100 μg/ml] and volume [2 or 4 ml]) or the reference treatment (hylan G-F 20). Patients were evaluated using Western Ontario McMaster Universities (WOMAC®) Scores and the Short-Form health survey up to 6 months post-injection (Month 6). Drug consumption and safety were evaluated.
Results:
Among 164 treated patients, 147 completed the study. The JTA-004 formulation containing 200 μg clonidine and 20 mg hyaluronic acid in 2 ml (JTA-200/2) was selected based on interim results at Month 6. The difference in adjusted mean change in WOMAC Pain Subscale Score from baseline (JTA-200/2 minus reference group) at Month 6 was − 9.49 mm; statistical superiority of JTA-200/2 over the reference was not demonstrated. No statistically significant differences in adjusted mean changes from baseline between JTA-200/2 and reference groups were observed for Pain, Physical Function and Stiffness Subscales WOMAC Scores, Total WOMAC Score, and Well-being Score at any timepoint, although JTA-200/2 induced larger improvements in WOMAC Scores than the reference. Statistically significantly larger improvements in WOMAC Pain Subscale Scores for JTA-004 versus the reference were observed in post-hoc analyses on pooled data from all JTA-004 formulations at Month 6 (p = 0.030) and Month 3 (p = 0.014). All JTA-004 formulations had clinically acceptable safety profiles.
Conclusions:
This study provided preliminary evidence of the safety of intra-articular injection of JTA-004 in knee osteoarthritis patients. Phase III randomized controlled trials with larger sample sizes are needed to evaluate the efficacy of JTA-004 in knee osteoarthritis.

A Phase IIa clinical trial for systemic lupus erythematosus ("SLE") had previously established safety for the patient and efficacy on the course of the autoimmune disease. Positive preclinical work gives good grounds for a Phase II clinical trial on systemic sclerosis ("SSc"). Phase IIb clinical trials for SLE and SSc are in the planning stage with the protocols for both studies being ready. It will only be possible to start the SLE and SSc Phase 2b clinical trials if the BioSenic Group succeeds in concluding a strong partnership with a biopharmaceutical company or if it manages to successfully out-license some of its technology. The start of SLE and SSc Phase II clinical trials is therefore not envisioned before 2025.Maximum cost reduction and disciplined cash management will remain a key priority, and the situation will be closely and actively monitored. The Company will actively initiate the search for new assets through M&A processes. PRESS RELEASE - REGULATED INFIRMATION BioSenic stopped the patient recruitment for its Phase 2b ALLOB trial in mid-2023. BioSenic has prepared a global restructuring plan covering the years 2024-2032 and will actively pursue follow-up actions. The Company indeed announced that it has used the statistical analysis capabilities of Artialis to study the results of the Phase 3 JTA-004 trial in the subset of patients with the most painful and inflammatory form of knee osteoarthritis (OA). This allows BioSenic to distinguish a group of patients, representing about one third of the total patients, who show a pain-relieving effect of JTA-004 not only superior to placebo but also to the active comparator. This new post-hoc analysis changes the therapeutic profile of the molecule and potentially allows for the possibility of stratifying patients for a new, optimized Phase 3 clinical study. In March 2023, BioSenic published new data on the mechanism of action of arsenic trioxide (ATO) to prevent autoimmune diseases in a peer-reviewed paper (Frontiers in Immunology). This new data shows that combination of ATO with copper salts can allow BioSenic to work towards reducing the dosage of ATO in future trials overall and maintain efficacy. This new formulation data has been completed following pre-clinical activities and does not constitute data validated through clinical trial.In April 2023, BioSenic appointed Lieven Huysse, MD, as permanent Chief Medical Officer (CMO).In April 2023, BioSenic received European patent from EPO, for further therapeutic development in cancer, infectious and immune diseases. The patent describes biomarkers to be used to determine if the condition of a patient worsens or improves following standard or new treatments for cGvHD. The Medsenic Phase 2 clinical study with arsenic trioxide in the first-line treatment of cGvHD has been completed and provided positive results. A Phase 3 study with oral arsenic trioxide in the first-line treatment of cGvHD, for which Medsenic received an encouraging pre-IND response from the FDA, is currently anticipated to start. A Phase 2a clinical trial for systemic lupus erythematosus ("SLE") had previously established safety for the patient and efficacy on the course of the autoimmune disease. Positive preclinical work gives good grounds for a Phase 2 clinical trial on systemic sclerosis ("SSc"). Phase 2b clinical trials for SLE and SSc are in the planning stage. BioSenic is currently preparing the best conditions for a successful fundraising. Outlook for the remainder of 2024
The detailed analysis of the Medsenic Phase 2 clinical study with arsenic trioxide in the first-line treatment of cGvHD has been completed and provides new valuable details for the next trial. These results will help justify clinically relevant choices for our forthcoming Phase 3 study with oral arsenic trioxide as a first-line treatment of cGvHD, for which Medsenic received earlier positive pre-IND responses from the FDA. A Phase 2a clinical trial for systemic lupus erythematosus (SLE) had previously established safety for the autoimmune patient and efficacy on the course of an autoimmune disease. Recent positive preclinical work gives good grounds for a Phase 2 clinical trial on systemic sclerosis ("SSc"). Phase 2b clinical trials for SLE and SSc are in the planning stage with the protocols for both studies being ready.BioSenic is currently preparing a fundraising composed of convertible debt and equity.
