Synergistic disruption of the blood-brain barrier in tree shrews by Methamphetamine and HIV-1 Tat via TRPM2-Mediated astrocyte inflammation

Methamphetamine (METH) abuse and HIV infection have emerged as two major public health concerns worldwide. Both METH and the HIV-1 Tat protein (Tat) can enter central nervous system through blood-brain barrier (BBB) and exert neurotoxic effects. However, the specific mechanism underlying their synergistic disruption of BBB remains unclear. Herein, co-culture models of endothelial cells and astrocytes and tree shrews were used to explore the role and mechanism of the Transient receptor potential M2 (TRPM2) in mediating the synergistic BBB damage induced by METH and Tat. In the tree shrew experiment, METH and/or Tat exhibited neurotoxicity, and the combination of them yielded more severe toxic effects characterized by tight junction protein loss, albumin leakage, astrocyte activation, neuroinflammation, and astrocytic TRPM2 activation. Similarly, in vitro experiments revealed that METH and Tat decreased tight junction proteins and transendothelial electrical resistance while increasing the flux of sodium fluorescein, and triggering astrocyte-mediated inflammation and astrocytic TRPM2 activation. However, pharmacological inhibition of TRPM2 mitigated the above-mentioned BBB damage and astrocyte-related inflammation both in vitro and in vivo. Furthermore, inhibition of TRPM2 mitigated METH + Tat-induced stereotypical behavior and anxiety-like behavior in tree shrews. Critically, astrocyte-specific TRPM2 knockdown in tree shrews effectively prevented METH and Tat-induced neuroinflammation and BBB damage, and ameliorated the associated behavioral impairments. This study revealed that METH and Tat could synergistically activate astrocytic TRPM2 channels to induce astrocyte-derived inflammatory response, and ultimately resulted in BBB injury. These findings highlight astrocyte-mediated inflammation as an important mechanism by which METH and Tat synergistically damage the BBB, and the TRPM2 channel represents a promising therapeutic target, providing novel insights into the mechanism of neurotoxicity caused by METH and HIV-1 Tat.

Experimental and observational evidence on substance use and metacognition among adults: a systematic review

PURPOSE:

Metacognition - the capacity to monitor, interpret, and regulate cognitive processes - is essential for adaptive behavioral regulation. Evidence on how substance use relates to metacognitive functioning remains heterogeneous across substances and study designs. This systematic review synthesized evidence examining the relationship between substance use and metacognition among adults.

METHODS:

Following PRISMA 2020 guidelines, we searched PubMed, PsycINFO, CINAHL, and Web of Science from inception through March 2026. Eligible studies included experimental and observational designs enrolling adults (≥18 years) and reporting metacognitive outcomes. Studies not assessing metacognition, involving non-adult samples, or non-empirical publications were excluded. Risk of bias was assessed using Joanna Briggs Institute appraisal tools. Findings were synthesized narratively.

RESULTS:

Thirty-six studies (N = 2,852 participants) were included: randomized controlled trials (n = 16), quasi-experimental (n = 3), cross-sectional (n = 7), and case-control studies (n = 10). Experimental evidence showed substance-specific effects: alcohol findings were mixed across paradigms; cannabinoids generally impaired metacognitive accuracy and calibration; benzodiazepines impaired metacognitive awareness of performance and error detection; methamphetamine demonstrated dose-dependent effects on agency judgments; and nicotine replacement did not restore accuracy following abstinence. Craving states (nicotine, caffeine, methamphetamine) were associated with poorer monitoring and calibration. Observational studies indicated that cocaine, nicotine, opioid, cannabis, alcohol, and polysubstance use were generally associated with poorer metacognitive functioning, including greater maladaptive metacognitive beliefs and reduced monitoring accuracy, though some clinical comparisons showed domain-specific or relatively preserved functioning.

CONCLUSION:

Experimental studies suggest acute substance-related alterations in metacognitive functioning, while observational studies indicate that substance use is generally associated with poorer metacognitive functioning. Greater standardization and longitudinal research are needed to clarify mechanisms and inform interventions.

Disruption of psychostimulant-associated memories by single, low dose ketamine in rats

Cocaine and methamphetamine use disorders pose public health concerns due to the high propensity for relapse driven by persistent drug-associated memories. Output from the medial prefrontal cortex (mPFC) regulates relapse and is controlled by parvalbumin (PV) interneurons, which are surrounded by perineuronal nets (PNNs) that support precise PV neuron firing. We previously showed that removal of mPFC PNNs, which reduces PV firing, disrupted reconsolidation of a cocaine self-administration memory when rats were given a novel, but not familiar, memory retrieval session. Ketamine rapidly suppresses PV neuron output, so we tested whether a single low dose (6 mg/kg) administered before or after a familiar or novel memory retrieval session altered cocaine cue reinstatement. Ketamine given prior to the novel, but not familiar, retrieval session reduced cue reinstatement in male Sprague-Dawley rats. In addition, both ketamine treatments combined with a novel retrieval session reduced PNN intensity around PV neurons after cue reinstatement, suggesting that PNN decreases may be unrelated to reinstatement. However, ketamine reduced the number of c-Fos-activated PV neurons immediately after the novel memory retrieval session, indicating that ketamine may reduce reinstatement through reduced PV neuron function at the time of memory retrieval. Ketamine given just prior to a novel memory retrieval session also reduced cue-induced reinstatement in rats trained for methamphetamine, but not fentanyl or sucrose self-administration. Collectively, these studies show that a single, low dose of ketamine combined with a novel memory retrieval reduces psychostimulant-seeking behavior, and may help delineate the mechanisms for ketamine-reduced relapse in psychostimulant use disorders.
