BALANCE-DM2: A Multiregional, Randomized, Multicenter, Active-Controlled Confirmatory Phase III Study to Evaluate the Efficacy and Safety of Bofanglutide (GZR18) in Latin American Adults With Type 2 Diabetes Mellitus

The goal of this clinical trial is to evaluate the efficacy and safety of Bofanglutide (GZR18) compared with Semaglutide in Latin American adults with type 2 diabetes mellitus who have inadequate glycemic control while receiving stable metformin monotherapy. The main questions it aims to answer are:
Does Bofanglutide (GZR18) provide glycemic control comparable to Semaglutide based on changes in HbA1c? Is Bofanglutide (GZR18) safe and well tolerated in the study population? Can participants achieve glycemic targets and improve metabolic outcomes during treatment?
Researchers will compare participants receiving Bofanglutide (GZR18) with participants receiving Semaglutide to evaluate their effects on glycemic control, metabolic outcomes, safety, quality of life, and treatment satisfaction.
Participants will:
Be randomly assigned in a 1:1 ratio to receive Bofanglutide (GZR18) or Semaglutide.
Continue stable metformin monotherapy during the study. Receive subcutaneous study treatment with dose escalation according to the study protocol.
Participate in 30 weeks of active treatment. Attend scheduled study visits, laboratory assessments, and safety evaluations. Complete questionnaires related to quality of life and treatment satisfaction.

Safety and efficacy of GZR18, a long-acting GLP-1 analog, in Chinese patients with type 2 diabetes: A randomized, double-blind, phase 1b/2a trial

GZR18 is a long-acting glucagon-like peptide-1 receptor agonist that is under development for the treatment of type 2 diabetes mellitus (T2DM). In this randomized, double-blind, placebo-controlled phase 1b/2a trial in Chinese adults with T2DM, the safety and efficacy of once-weekly GZR18 are evaluated using different dose-titration regimens in two separate parts: Part A (0.5-4 mg over 26 weeks) and Part B (1.5-13 mg over 23 weeks). Overall, 72 participants are enrolled, of whom 62 complete the trial. GZR18 significantly reduces glycated hemoglobin (HbA1c) in both parts (Part A: -1.32% for GZR18 versus 0.86% for placebo; Part B: -1.81% for GZR18 versus 0.12% for placebo). Improvements in body weight and other metabolic parameters are also observed. GZR18 is safe and well tolerated, with mostly mild-to-moderate gastrointestinal adverse events; no severe hypoglycemia or treatment-related serious adverse events occurred. These findings support further development of GZR18 in larger, longer-term trials. The trial is registered at ClinicalTrials.gov (NCT06256523).

GZR18, a GLP-1 analog with once-weekly or bi-weekly dosing for body weight management: A randomized, placebo-controlled, phase 1b/2a trial

GZR18 is a glucagon-like peptide-1 receptor agonist under development for overweight/obesity. In this randomized, placebo-controlled phase 1b/2a trial in Chinese adults with overweight or obesity, different dose titration regimens and dosing frequencies (once-weekly [QW] vs. bi-weekly [Q2W]) are evaluated to assess safety and weight-loss efficacy over 26 weeks (Part A) or 35 weeks (Part B). Sixty participants are enrolled, of whom 46 complete the trial. The least-squares mean change in body weight is -9.36% for GZR18 vs. 6.68% for placebo in Part A and -17.8% (QW) and -12.8% (Q2W) for GZR18 vs. 0.7% for placebo in Part B; GZR18 also improves other weight-related and metabolic parameters. GZR18 is generally safe and well tolerated, with mostly mild-to-moderate gastrointestinal adverse events and no investigational-product-related serious adverse events reported. These findings warrant larger and longer trials of GZR18 for body weight management. The trial is registered at ClinicalTrials.gov (NCT06256536).

The safety, tolerability, pharmacokinetics and pharmacodynamics of GZR18 in healthy American and Chinese adult subjects

Abstract:

Aims:

GZR18, a novel long‐acting GLP‐1 receptor agonist, has demonstrated substantial metabolic improvements in diabetic and obese animal models. The present studies aimed to evaluate the safety, tolerability, pharmacokinetics (PK) and pharmacodynamics (PD) of the ascending dose of GZR18 in healthy American and Chinese subjects.

Materials and Methods:

In these phases 1, randomized, double‐blind, placebo‐controlled, sequential, dose‐escalation US and Chinese studies, healthy American and Chinese adults with similar age were enrolled to once‐weekly subcutaneous injection of GZR18 or placebo. The studies included three cohorts of male American subjects (cohorts US‐1–3) and six cohorts of Chinese subjects (cohorts CN‐1–6, male and female), each with a specified target dose of GZR18 ranging from 1 to 50 μg/kg (1–10 μg/kg for US study and 5–50 μg/kg for Chinese study). The primary endpoints were the safety and tolerability of GZR18. Blood samples were collected for PK and PD analysis of GZR18 before and after dosing. A population PK analysis of GZR18 was conducted to ascertain whether there are ethnic PK differences between American and Chinese adults.

Results:

The exposure of GZR18 was comparable between healthy American and Chinese subjects, with the geometric mean ratio between the two populations for AUC 0‐t and Cmax close to 1. A dose‐dependent increase in AUC 0–t and Cmax occurred in both populations. The median time to maximum plasma concentrations ( Tmax ) in American subjects ranged from 72 to 96 h, and the mean Tmax ranged from 60 to 72 h in Chinese subjects. The half‐life of GZR18 was approximately 7 days in both American and Chinese subjects. Evident body weight reduction was observed in GZR18 treatment groups in Chinese subjects (cohorts CN‐3–6 on Day 15, −1.25 to −1.86 kg; −1.88% to −3.11%). No deaths, serious adverse events or hypoglycaemia were reported. Decreased appetite and nausea were the most frequently reported treatment‐emergent adverse events, observed in Chinese study and mild in severity. The safety profile of GZR18 was generally consistent with the same class of drugs.

Conclusions:

GZR18 demonstrates good tolerability in healthy American and Chinese subjects. No ethnic differences were observed between healthy American and Chinese subjects. The safety, PK and PD profiles of GZR18 support its further clinical evaluation for glycaemic and body weight control.

Bofanglutide Linked to Meaningful Reductions in HbA1c in Adults With T2DM

THURSDAY, July 2, 2026 -- For adults with type 2 diabetes mellitus (T2DM), bofanglutide, a glucagon-like peptide-1 receptor agonist, is associated with meaningful reductions in hemoglobin A1C (HbA1c) levels compared to semaglutide, according to a study published online June 30 in the Annals of Internal Medicine . Ming Liu, M.D., from Tianjin Medical University General Hospital in China, and colleagues examined the efficacy and safety of bofanglutide versus semaglutide for T2DM and overweight and obesity in a phase 2b multicenter randomized trial. A total of 272 adults with T2DM who were drug-naive or receiving stable oral antidiabetic drugs were randomly assigned to bofanglutide titrated to targets of 12, 18, or 24 mg (biweekly [Q2W]); bofanglutide titrated to 24 mg (once weekly [QW]); or semaglutide titrated to 1 mg QW. The researchers found that the change in HbA1c level from baseline to week 24 was −1.87, −2.28, and −1.94 percent for bofanglutide 12, 18, and 24 mg Q2W, respectively; −2.32 percent for bofanglutide 24 mg QW; and −1.60 percent for 1 mg semaglutide QW. Compared with semaglutide, the corresponding treatment differences were −0.27, −0.68, −0.34, and −0.72 percent, respectively. In the bofanglutide and semaglutide groups, gastrointestinal adverse events (mostly grade 1 or 2 in severity) occurred in 81.8 to 87.3 percent and in 51.9 percent, respectively. "In this phase 2b trial involving Chinese patients with T2DM, 24-week treatment with bofanglutide showed meaningful glycemic control, weight loss, and improvements in various cardiometabolic risk factors relative to pretreatment baseline," the authors write. "These reductions seemed at least as large or larger when compared with 1 mg of semaglutide QW, the highest approved dose for T2DM, in alignment with clinical practice." Several authors disclosed ties to Gan & Lee Pharmaceuticals, which funded the study.
