A Phase II Study of Immunotherapies (Tiragolumab and Atezolizumab) in Combination With Stereotactic Body Radiation Radiotherapy in Microsatellite Stable (MSS) Metastatic Colorectal Cancer (mCRC)

Background:
Metastatic colorectal cancer (mCRC) is cancer that has spread beyond the colon and rectum. Most people with mCRC die within 5 years. New immune-based treatments are making progress with some types of colon cancer. But these treatments do little for people with a type of cancer that is microsatellite stable (MSS). MSS is a specific cancer biomarker. Better treatments are needed.
Objective:
To test 2 drugs (tiragolumab and atezolizumab) combined with radiation therapy in people with MSS mCRC.
Eligibility:
People aged 18 years and older with MSS mCRC.
Design:
Participants will be screened. They will have a physical exam with blood tests. They will have imaging scans and a test of their heart function. They will provide a tissue sample from their tumor; if one is not already available, a new sample will be taken. Their ability to perform normal tasks will be assessed.
Tiragolumab and atezolizumab are both administered through a tube attached to a needle inserted into a vein. Participants will receive both drugs on day 1 of 3-week treatment cycles. Each study visit should last about 8 hours.
Participants will receive radiation therapy on days 1, 3, and 5 of cycle 1 only.
Blood samples and rectal swabs will be collected on day 1 of every cycle.
Imaging scans will be repeated every 9 weeks. Additional tumor samples may be taken during treatment.
Treatment will continue for up to 2 years.
Participants will have a follow-up visit 1 month after treatment ends. Follow-up visits will continue every 3 months for 1 more year.

Phase 2 and Biomarker Trial of Anti-TIGIT and Anti-PDL1 in Patients With Recurrent Glioblastoma

This phase II trial compares the safety, side effects and effectiveness of atezolizumab with tiragolumab to atezolizumab alone in treating patients with glioblastoma that has come back after a period of improvement (recurrent). Glioblastoma is the most common primary brain cancer in adults and despite aggressive treatment, it is nearly always fatal. Currently, there are limited effective treatment options in patients that have recurrence. Immunotherapy has been shown to be effective in other types of cancer and may be an appealing potential treatment option for recurrent glioblastoma. Immunotherapy with monoclonal antibodies, such as atezolizumab and tiragolumab, may help the body's immune system attack the cancer, and may interfere with the ability of tumor cells to grow and spread. Study doctors also want to learn if a tumor infiltrating T lymphocyte (TIL) response is helpful to determine the benefit of the combination of study drugs compared to the usual approach. TILs are a type of immune cell that has moved from the blood into a tumor. TILs can recognize and kill tumor cells. Giving atezolizumab with tiragolumab may be safe, tolerable and/or effective compared to atezolizumab alone in treating patients with recurrent glioblastoma.

Investigation of excipient-related artifacts observed in the leachables study of tiragolumab by LC-HRMS and GC-MS

Leachables studies of protein formulations are faced with many analytical challenges arising from relatively high levels of complex formulation components. In the stressed sample of the monoclonal antibody drug tiragolumab with the main excipients methionine, histidine, sucrose, and polysorbate, two unknown compounds were detected in the screening for non-volatile leachables by reversed phase (RP) liquid chromatography high-resolution mass spectrometry (LC-HRMS). Unknown 1 was proven to be a gas-phase mass spectral adduct between methionine and the sucrose-derived hexose. Separation of methionine and hexose by hydrophilic interaction chromatography (HILIC) resulted in the disappearance of the unknown peak. It was also observed that non-covalent amino acid-sugar adducts in the gas phase were prevalent in both electrospray ionization (ESI) and atmospheric pressure chemical ionization (APCI) modes. Unknown 2 was identified as a Maillard reaction product 1-(3H-imidazo[4,5-c]pyridin-4-yl)ethan-1-one by LC-HRMS and gas chromatography mass spectrometry (GC-MS). The formation mechanism was confirmed by the reaction between histidine and the sucrose-derived methylglyoxal (MGO) under hydrothermal conditions. This study exemplifies false positive leachable results and delves into the chemical reactions and interactions that generated such artifacts.

Fc-enhanced anti-TIGIT antibody 30,278-IgG1 RMD amplifies antitumor immunity through effector cell activation and synergizes with PD-1 blockade

TIGIT is an inhibitory immune checkpoint receptor, and its blockade has shown clinical promise in combination with PD-1 pathway inhibitors. However, whether anti-TIGIT antibodies should engage immune effector functions via Fcγ receptors (FcγRs) remains an open question. We developed 30,278-IgG1 RMD, a novel Fc-enhanced anti-TIGIT antibody, to amplify effector cell activation through FcγR engagement while preserving TIGIT binding and blockade. 30,278-IgG1 RMD was generated by Fc glycoengineering to increase affinity for activating FcγRs. This variant was compared to wild-type IgG1, an Fc-inert IgG4, and tiragolumab in in vitro assays (TIGIT binding/blockade, ADCC, ADCP, Treg depletion, immune cell activation in human PBMCs) and in an hTIGIT/hPD-1 knock-in mouse colon carcinoma model (CT26) combined with PD-1 blockade. 30,278-IgG1 RMD maintained high TIGIT affinity and blockade activity, while exhibiting markedly increased binding to activating FcγRs. It triggered more potent ADCC and ADCP than wild-type or tiragolumab, resulting in efficient depletion of TIGIT⁺ Tregs and activation of NK cells and dendritic cells. In human PBMC assays, the Fc-enhanced antibody augmented T cell activation and cytokine production relative to Fc-silent and wild-type controls. In vivo, 30278-IgG1 RMD plus PD-1 blockade yielded superior tumor control, including complete tumor regressions in some mice, whereas Fc-inert or wild-type anti-TIGIT combinations did not. Fc engineering of an anti-TIGIT antibody substantially improves immune effector engagement and anti-tumor efficacy. Augmenting FcγR interactions alongside checkpoint blockade can potentiate T cell responses and drive tumor regression, underscoring the translational potential of Fc-optimized checkpoint immunotherapies.

Importance:

Programmed cell death 1 ligand 1/programmed cell death protein 1 inhibitors, with or without chemotherapy, are standard first-line treatment for patients with advanced non-small cell lung cancer (NSCLC); however, survival benefit is limited, and many patients experience disease progression.

Objective:

To evaluate the efficacy and safety of tiragolumab plus atezolizumab plus chemotherapy vs placebo plus pembrolizumab plus chemotherapy in patients with advanced nonsquamous NSCLC.

Design, Setting, and Participants:

SKYSCRAPER-06 was a phase 3 randomized clinical trial that recruited patients with previously untreated, locally advanced unresectable or metastatic NSCLC at 129 sites in 21 countries between December 15, 2020, and September 14, 2023 (data cutoff, April 19, 2024).

Intervention:

Patients were randomized 1:1 to receive either tiragolumab, 600 mg, plus atezolizumab, 1200 mg, plus chemotherapy (pemetrexed, 500 mg/m2, and carboplatin [area under the curve 5], or cisplatin, 75 mg/m2) or placebo plus pembrolizumab, 200 mg, plus chemotherapy via intravenous infusion on day 1 of each 21-day cycle until disease progression, loss of clinical benefit, unacceptable toxic effect, or withdrawal of consent.

Main Outcomes and Measures:

Primary end points were investigator-assessed progression-free survival and overall survival. The safety and tolerability of the study drugs were also evaluated.

Results:

Of 542 patients in the full analysis set (mean [SD] age, 63.6 [9.3] years; 353 [65.1%] male), 269 were randomized to tiragolumab plus atezolizumab plus chemotherapy and 273 to placebo plus pembrolizumab plus chemotherapy. Overall, baseline demographics were similar between treatment groups. At data cutoff (median follow-up, 11.8 months), median investigator-assessed progression-free survival was 8.3 months (95% CI, 7.1-9.6 months) with tiragolumab plus atezolizumab plus chemotherapy vs 9.9 months (95% CI, 8.7-11.9 months) with placebo plus pembrolizumab plus chemotherapy (hazard ratio, 1.27; 95% CI, 1.02-1.57; P = .99); median overall survival was 18.9 months (95% CI, 15.2-23.8 months) vs 23.1 months (95% CI, 20.7-33.0 months) in each treatment group, respectively (hazard ratio, 1.33; 95% CI, 1.02-1.73; P = .98). Grade 3 to 4 adverse events occurred in 164 of 267 patients (61.4%) in the tiragolumab plus atezolizumab plus chemotherapy group and 165 of 272 patients (60.7%) in the placebo plus pembrolizumab plus chemotherapy group, with grade 5 AEs occurring in 27 of 267 patients (10.1%) and 16 of 272 patients (5.9%) in each group, respectively.

Conclusions and Relevance:

In the phase 3 SKYSCRAPER-06 randomized clinical trial, the primary end points were not met and the study has been terminated.

Tecentriq Filed for Additional Indication as Maintenance Therapy Following Definitive Chemoradiotherapy in Locally Advanced Esophageal Cancer

If approved, Tecentriq is expected to become the first immune checkpoint inhibitor for maintenance therapy in patients with locally advanced esophageal cancer whose disease has not progressed following definitive chemoradiotherapy Filing based on results from the global Phase III clinical trial (SKYSCRAPER-07) TOKYO, June 12, 2026 -- Chugai Pharmaceutical Co., Ltd. (TOKYO: 4519) today announced that it has filed a regulatory application with the Ministry of Health, Labour and Welfare for the anti-cancer agent/humanized anti-PD-L1 monoclonal antibody Tecentriq® Intravenous Infusion 1200 mg [generic name: atezolizumab (genetical recombination)] for an additional indication as maintenance therapy following definitive chemoradiotherapy in locally advanced esophageal cancer. “There is no established standard treatment for maintenance therapy following definitive chemoradiotherapy for locally advanced esophageal cancer. Gilead, Arcus tumble again in TIGIT, triggering 2 trial culls

iStock, dickcraft The Phase 3 failure has prompted Gilead Sciences and Arcus Biosciences to terminate a mid-stage study of their TIGIT asset in lung cancer. Gilead Sciences and Arcus Biosciences are discontinuing a late-stage study of their investigational TIGIT drug domvanalimab “due to futility,” prompting Gilead to start rolling back its years-long cancer collaboration with Arcus. The trial, STAR-121, was testing domvanalimab in combination with a PD-1 blocker and chemotherapy for metastatic non-small cell lung cancer (NSCLC). Arcus didn’t provide specific data from the study in the legal filing, noting only that an independent data panel found the comparison against Merck’s Keytruda plus chemotherapy to be futile. No new safety issues were found during regular check-ins with the data board, though the latest futility analysis did not include a safety assessment. Arcus made the disclosure in a securities filing dated April 20, noting that Gilead has decided not to make an option continuation payment, a decision that will cut off the pharma’s access to a clutch of early-stage TIGIT therapies. Gilead’s opportunity to participate in the development of new Arcus early-stage programs will officially end on July 14. Gilead, however, will retain its “existing time-limited options” to certain programs, including the small-molecule AXL blocker AB801, the anti-CD39 antibody AB598 and the MRGPRX2 antagonist AB102. Initially, the partners saw some success as other TIGIT players struggled. In June 2024, for instance, long-term data from the Phase 2 EDGE-Gastric study showed a 58.5% overall response rate in patients with upper gastrointestinal cancers after treatment with domvanalimab plus an anti-PD-1 therapy. In December last year, a domvanalimab regimen flopped in a late-stage study of gastric and esophageal cancer, forcing the partners to abandon their plans in the indication. The TIGIT tale has been rife not just with clinical failures but also with sunk upfront costs. Immutep’s LAG3 drug eftilagimod alfa halted in Phase III for NSCLC due to futility

Sydney-based Immutep Limited announced that the Independent Data Monitoring Committee for its TACTI-004 Phase III trial recommended discontinuation of the study evaluating eftilagimod alfa in combination with pembrolizumab and chemotherapy as first-line treatment for advanced or metastatic non-small cell lung cancer. The recommendation followed a planned interim futility analysis, indicating the addition of eftilagimod alfa to standard pembrolizumab-chemotherapy was unlikely to demonstrate a treatment benefit over the control arm. TACTI-004 was a randomized, double-blind, controlled Phase III study designed to enroll approximately 756 patients across more than 150 sites in over 25 countries. Eligible patients had advanced or metastatic NSCLC without EGFR, ALK, or ROS1 genomic aberrations and were enrolled regardless of PD-L1 expression status. Patients were randomized 1:1 to receive either eftilagimod alfa plus pembrolizumab plus chemotherapy, or pembrolizumab plus chemotherapy plus placebo. The dual primary endpoints were progression-free survival and overall survival. The IDMC reviewed available safety and efficacy data at the prespecified interim analysis and concluded the trial met criteria for futility. Immutep did not disclose specific efficacy or safety results from the analysis. The company said enrollment has been halted and an orderly wind-down is underway, including patient follow-up and site closure in accordance with regulatory obligations. Immutep stated it is conducting a review of the available data to determine next steps for the eftilagimod alfa program. The company noted that discontinuation of TACTI-004 extends its anticipated cash runway beyond the previously guided Q2 2027 timeframe. Eftilagimod alfa holds US FDA Fast Track designation in both first-line NSCLC and first-line head and neck squamous cell carcinoma, and remains under evaluation in other solid tumor indications. Immutep said it will reassess capital allocation once its operational and data reviews are complete. Eftilagimod alfa is a soluble LAG-3 immunoglobulin fusion protein that functions as an MHC Class II agonist. Rather than blocking an inhibitory checkpoint, it binds MHC Class II molecules on antigen-presenting cells such as dendritic cells and monocytes, activating them to prime cytotoxic T cells and generate co-stimulatory signals intended to amplify anti-tumor immunity. The rationale for combining eftilagimod alfa with pembrolizumab, a PD-1 checkpoint inhibitor, was to simultaneously release T cell inhibition and enhance antigen presentation. The futility finding suggests this dual mechanism did not translate into additional clinical benefit over pembrolizumab and chemotherapy alone in this patient population. First-line treatment for advanced NSCLC without actionable driver mutations is dominated by PD-1 or PD-L1 checkpoint inhibitors combined with platinum-based chemotherapy. Several programs are testing whether additional immune modulation can improve upon this standard. Key competitors include: Roche’s tiragolumab, an anti-TIGIT antibody evaluated in combination with atezolizumab in the Phase III SKYSCRAPER-01 trial for PD-L1-high first-line NSCLC. Arcus Biosciences’ domvanalimab, another anti-TIGIT antibody being studied with the PD-1 inhibitor zimberelimab in the Phase III ARC-10 trial in first-line NSCLC. AstraZeneca and Daiichi Sankyo’s datopotamab deruxtecan, a TROP2-directed antibody-drug conjugate in the Phase III TROPION-Lung07 trial testing combination with pembrolizumab against pembrolizumab plus chemotherapy in first-line NSCLC. The TACTI-004 result narrows Immutep’s late-stage pipeline and raises questions about whether MHC Class II agonism via LAG-3 engagement can produce measurable efficacy gains in large randomized trials. Of note, clinicaltrials.gov indicates Immutep is continuing to study eftilagimod alfa in the Phase II/III AIPAC-003 trial focused on metastatic breast cancer.
