A Phase I Non-Randomized Study of TP53 Reactivation With Arsenic Trioxide in Allogeneic Transplantation for Acute Myeloid Leukemia and Myelodysplastic Syndrome

This phase I trial tests the safety, side effects, best dose and how well arsenic trioxide works to prevent relapse in patients undergoing allogenic stem cell transplantation for acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS). Chemotherapy drugs, such as arsenic trioxide, work in different ways to stop the growth of cancer cells, either by killing the cells, by stopping them from dividing, or by stopping them from spreading. Giving arsenic trioxide may be safe, tolerable and/or effective in preventing relapse in patients undergoing allogenic stem cell transplant for AML or MDS.

A Phase 1 Study of ATRA and SDK002 in Combination With Chemotherapy and Anti-PD-L1 Inhibitor in Patients With Advanced Pancreatic Ductal Adenocarcinoma.

This study is testing whether adding three drugs, All-Trans Retinoic Acid (ATRA), SDK002 (also called Arsenic Trioxide or ATO), and tislelizumab, to standard chemotherapy is safe for people with advanced pancreatic cancer. Advanced pancreatic cancer means the cancer has spread or cannot be removed with surgery. The study will also look at whether this treatment combination may help people live longer.
Participants will receive standard chemotherapy drugs, gemcitabine and nab-paclitaxel, together with ATRA, SDK002, and tislelizumab. ATRA is related to vitamin A and may affect how cancer cells grow. SDK002 is a drug used to treat sine cancers and may help other treatments work better. Tislelizumab is an immunotherapy drug, which helps the immune system recognize and attack cancer cells. This is a phase 1 study, which means the main goal is to test safety and side effects. All participants receive the same treatment, and both the study doctors and participants know which drugs are being given.

Acute leukemia during pregnancy: a single center experience with 23 cases, 2012–2022 and literature review

BACKGROUND AND OBJECTIVE:

Acute Leukemia (AL) diagnosed during pregnancy is uncommon, and the management of AL complicated by pregnancy is a poorly studied area that faces a therapeutic dilemma.

METHOD:

23 cases diagnosed with AL during pregnancy were collected from our center between 2012 and 2022. Additionally, we summarized 17 clinical studies on AL diagnosed during pregnancy.

RESULT:

Sixteen AML and seven ALL patients were diagnosed during pregnancy in our center; most patients diagnosed during the first or second trimester tended to receive chemotherapy after abortion. All patients diagnosed during the third trimester opted to defer therapy until after delivery. The rate of overall response rate (ORR) was 77.3%, which was found to be similar to that of the general population. A Short delay in initiating anti-leukemia treatment is not associated with a negative impact on prognosis. We conducted a meta-analysis of the literature, and the summary relative complete remission (CR) rate was 76.9% (95%CI 69.1-84.6%) (Random Effects Model I^2 = 66%, p <0.001). We included four Chinese studies comprising 135 participants for further analysis. For non-M3 AL patients, CR rate of the chemotherapy-first cohort was 63.6%, compared with 68.6% in the treatment-after-delivery or abortion cohort (p = 0.657). For M3 patients, the rate was 100 and 83.3%, respectively (p < 0.001).

CONCLUSION:

Our findings suggest that for non-M3 AL patients can sometimes be managed during pregnancy. For M3 patients, early treatment potentially yield favorable outcomes for both the mother and fetus, using ATRA-based regimens without arsenic trioxide (in the second and third trimester) or standard chemotherapy (in the second trimester) may be considered a treatment option.

Association between usual protein intake and muscle function in older U.S. adults: a target-trial emulation using NHANES 2011–2018

BACKGROUND:

The link between long-term protein intake and muscle performance in older adults has been hard to define, partly because most studies rely on short dietary windows and are vulnerable to confounding and measurement noise. In this work, we attempted to estimate the usual protein intake and functional limitation among U.S. adults aged ≥ 60 years using a target-trial emulation framework with overlap weighting and semiparametric estimators.

METHODS:

Data were drawn from four NHANES survey cycles (2011-2018), including 5,736 adults aged ≥ 60 years with complete exposure, outcome, and covariate data. Usual protein intake (g/kg/day) was derived from available 24-hour recalls to approximate habitual intake. The primary outcome was PFQ-defined mobility limitation across cycles; grip strength (2011-2014) was analyzed separately as a secondary outcome. Causal contrasts across predefined intake categories (<0.8, 0.8- < 1.0, 1.0- < 1.2, ≥1.2 g/kg/day) were evaluated using covariate-balancing propensity score overlap weighting (ATO estimand) followed by marginal structural models. Doubly robust sensitivity analyses were conducted using augmented inverse probability weighting and targeted maximum likelihood estimation with generalized linear models. Simulation extrapolation (SIMEX) was applied to assess potential bias from dietary measurement error. Exploratory analyses evaluated hs-CRP as a potential mediator and tested effect modification by vitamin D status and physical activity.

RESULTS:

Mean usual protein intake was 0.93 g/kg/day, and approximately 42% of participants consumed at least 0.8 g/kg/day, the current Recommended Dietary Allowance (RDA) for the general adult population. In the prespecified overlap-weighted marginal structural model (ATO estimand), higher intake was associated with lower odds of mobility limitation, although the primary contrast comparing ≥ 1.2 versus < 0.8 g/kg/day was modest and not statistically significant (OR 0.89, 95% CI 0.54-1.47). A doubly robust binary contrast yielded a -6.6 percentage-point difference in predicted limitation (95% CI -25.8 to 12.7), consistent in direction but imprecise. In cycle-specific analyses, the inverse association was more pronounced in 2015-2018 (OR 0.80, 95% CI 0.65-0.98). Spline models suggested a steeper decline in predicted limitation below approximately 1.0-1.1 g/kg/day, with a flatter trajectory at higher intakes. Exploratory mediation models indicated a potential indirect component through hs-CRP, though these estimates were not overlap-weighted and should be interpreted cautiously.

CONCLUSIONS:

Higher usual protein intake was directionally associated with lower odds of mobility limitation among older U.S. adults within a target trial emulation framework, although the primary overlap-weighted estimates were modest and imprecise. Evidence of nonlinearity suggests that intakes near 1.0-1.1 g/kg/day may mark a range where predicted limitation declines more steeply, but uncertainty increases at higher intake levels. Given the cross-sectional design and residual potential for confounding, these findings should be interpreted cautiously. Prospective studies are needed to determine whether sustained protein intake in this range meaningfully preserves functional capacity over time.

Co-exposure to fluoride and arsenic induces endoplasmic reticulum stress in C2C12 myoblasts and MC3T3 preosteoblasts

Environmental exposure to fluoride and arsenic is a major public health concern in many regions worldwide. Although both toxicants are known to impair skeletal development, the cellular mechanisms underlying their combined effects on skeletal muscle and bone cells remain poorly understood. This study investigated the impact of sodium fluoride (NaF) and arsenic trioxide (As₂O₃) co-exposure, for the first time, in C2C12 myoblasts (at 0.14 mM NaF + 2.4 µM As₂O₃) and MC3T3 preosteoblasts (at 0.64 mM NaF + 13 µM As₂O₃). Co-exposure significantly reduced cell proliferation and decreased the expression of the lineage specific transcription factors such as MYOD and RUNX2 in C2C12 and MC3T3 cells, respectively. Both individual and combined treatments increased ROS production, with co-exposure producing the highest oxidative stress. Further, fluoride and arsenic co-exposure exerted its effects by inducing endoplasmic reticulum stress in both cell types which was witnessed by the activation of the unfolded protein response (UPR) pathways. Specifically, in C2C12 myoblasts, co-exposure predominantly activated the PERK-Atf4-Chop pathway while suppressing IRE1α-Xbp1 expression, indicating unresolved ER stress. In contrast, MC3T3 preosteoblasts activated both PERK-Atf4-Chop and IRE1α-Xbp1 pathways, suggesting engagement of adaptive ER stress mechanisms. Collectively, these findings demonstrate that fluoride and arsenic co-exposure disrupts cellular homeostasis by inducing ER stress in both myoblasts and preosteoblasts. The differences in activation of different UPR pathways suggests C2C12 myoblasts to be more susceptible to fluoride and arsenic co-exposure compared to MC3T3 preosteoblasts.
