RATIONALE: Vaccines may make the body build an immune response to kill tumor cells. Drugs used in chemotherapy use different ways to stop tumor cells from dividing so they stop growing or die. Combining vaccine therapy with chemotherapy may kill more tumor cells.
PURPOSE: Phase II trial to study the effectiveness of combining vaccine therapy and chemotherapy in treating patients who have metastatic or locally recurrent stomach cancer or esophageal cancer.

Exogenous carbohydrate form during low-muscle glycogen conditions has minimal impact on cycling performance

BACKGROUND:

Skeletal muscle recovery is improved with immediate postexercise carbohydrate feeding. Little is known regarding muscle recovery and performance when feeding is delayed. The purpose of this study was to examine the effects of varying exogenous carbohydrates on cycling performance with delayed feedings and low skeletal muscle glycogen content.

METHODS:

Following 60 min of cycling and an overnight fast (12.1 ± 0.4 h), the participants consumed 2.10 ± 0.13 g·kg^-1 bodyweight carbohydrate of one of the following: whole potatoes (POT), cooked pasta (PAS), energy gel (GEL) or remained unfed (control, CON), then rested for 120 min. The participants then cycled for 60 min at 65% W_max and completed a 19.4 km time trial. Muscle and blood samples were collected prefeeding, 120 min postfeeding, and after cycling for glycogen, glucose, and insulin analyses.

RESULTS:

The time trial mean power output was higher in the PAS (213 ± 56W, p = 0.006) and GEL (209 ± 71W, p = 0.011) compared to CON (179 ± 68W), but POT (196 ± 51W, p = 0.199) was not different from CON. Power was similar between POT, PAS, and GEL (p > 0.05). Time trial finish time trended towards significance (p = 0.088) with carbohydrate trials averaging 3 min faster than CON. Muscle glycogen was similar between trials (p = 0.446) and did not change due to feeding (prefeeding: 44 ± 21 mmol·kg^-1, postfeeding: 47 ± 23 mmol·kg^-1, p = 0.120). Glycogen declined after cycling for 60 min (26 ± 16 mmol·kg^-1, p < 0.001) compared to pre-feeding and post-feeding samples. Glucose and insulin were elevated in carbohydrate trials over CON 0-30 min post-feeding (p < 0.05).

CONCLUSIONS:

Varied pre-exercise exogenous carbohydrate sources effectively improve cycling time trial performance in a glycogen compromised state.

Novel lytic phage vB-Vp-ZX1018 potentiates ofloxacin activity against multidrug-resistant Vibrio parahaemolyticus infections

The increasing prevalence of multidrug-resistant (MDR) Vibrio parahaemolyticus poses a growing threat to public health and aquaculture, underscoring the need for alternative therapeutic strategies. In this study, a clinically derived MDR Vibrio parahaemolyticus strain (Vp1018) was characterized; its genome encodes multiple antimicrobial resistance determinants and exhibits clear pathogenicity despite lacking the canonical virulence genes tdh and trh. Using Vp1018 as the host, a lytic phage, vB-Vp-ZX1018, was isolated and identified as a Siphoviridae (genus Mardecavirus) double-stranded DNA phage with a high burst size, strong physicochemical stability, and no detectable virulence or resistance genes, supporting its potential as a therapeutic antibacterial agent. In vitro bactericidal assays demonstrated marked phage-antibiotic synergy (PAS) between vB-Vp-ZX1018 and ofloxacin (OFL), in which low phage inputs combined with sub-inhibitory antibiotic concentrations achieved effective bacterial suppression while substantially reducing the required antibiotic dose. In vivo, PAS markedly improved therapeutic outcomes. In the Galleria mellonella model, the combination increased survival from 0% to 80% and reduced the EC₅₀ of OFL by ∼15.85-fold compared with antibiotic monotherapy. These benefits were further validated in the mouse oral infection model, where combination treatment produced the highest survival rates, substantially lowered intestinal and fecal bacterial burdens, and markedly alleviated histopathological injury relative to either agent alone. Taken together, our data indicate that vB-Vp-ZX1018 represents a safe therapeutic candidate and that rationally optimized PAS may offer an effective strategy for the treatment of MDR Vibrio parahaemolyticus infections.

Proanthocyanidins: structure, biosynthesis, regulation, and structure–activity relationships

Proanthocyanidins (PAs) are polyphenolic compounds widely distributed throughout the plant kingdom, playing critical biological and ecological roles, including in seed dormancy and defense. Owing to their powerful antioxidant, protein-binding, and antimicrobial properties, PAs also have broad applications in healthcare, the food industry, and animal nutrition. Here, we examine recent advances in PA research, focusing on their structural diversity, biosynthetic pathways, regulatory networks, and potential applications. All PAs are oligomers or polymers of flavan-3-ol monomers. PA biosynthesis begins with the phenylpropanoid pathway and the production of the starter units (-)-epicatechin and (+)-catechin. Polymerization proceeds by the addition of activated extension units through non-enzymatic mechanisms. PA biosynthesis is facilitated by metabolic compartmentation and precisely regulated by the MYB-bHLH-WD40 core transcriptional complex, which integrates signals from various phytohormones and environmental factors. Despite sharing common building blocks, PAs are structurally extremely complex, due to variations in their constituent monomers, degree of polymerization, linkage patterns, and chemical modifications. These structural features of PAs collectively determine their physicochemical properties and biological activities. Future PA research should focus on key knowledge gaps, including the site(s) of polymerization within the cell, the mechanisms that determine polymer structure, and the precise structure-activity relationships of PAs. Combined with advanced purification technologies and gene editing and synthetic biology strategies, this knowledge will allow the precise biomanufacturing of target PAs, thereby advancing both fundamental research and industrial applications.
