Impact of diabetes on the effects of SGLT2 inhibitors on kidney outcomes: An updated drug/dose-dependent meta-analysis

BACKGROUND:

Sodium-glucose co-transporter-2 (SGLT2) inhibitors provide renal and cardiovascular benefits in diabetes, but their effects in non-diabetic populations remain unclear. This meta-analysis evaluates the renal outcomes of SGLT2 inhibitors vs. placebo, focusing on diabetes status, subgroup variations, and drug-specific effects.

MATERIALS AND METHODS:

A systematic review of 31 randomized controlled trials (RCTs) (98,516 patients) was conducted. Studies including diabetic and non-diabetic patients were analyzed, with subgroup assessments based on diabetes status, drug type, dose, baseline estimated glomerular filtration rate (eGFR), chronic kidney disease (CKD) stage, and follow-up duration. Primary outcomes included kidney disease progression, while secondary outcomes encompassed renal adverse events, composite renal outcomes, acute kidney injury (AKI), diabetic ketoacidosis (DKA), and renal failure.

RESULTS:

SGLT2 inhibitors reduced progressive kidney disease risk in diabetic (OR = 0.64, 95% CI: 0.58 - 0.71) and non-diabetic patients (OR = 0.69, 95% CI: 0.57 - 0.83), with no effect modification by diabetes status (p = 0.49). Among diabetics, risk reductions were notable for canagliflozin 100 mg, dapagliflozin 10 mg, and empagliflozin 10-mg, particularly in patients with CKD stage 2 - 3 and baseline eGFR of 30 - 90 mL/min/1.73m². Among non-diabetics, dapagliflozin and empagliflozin showed consistent benefit. No differences in renal adverse events were observed in either group. DKA risk was elevated in diabetics receiving SGLT2 inhibitors (OR = 2.18, 95% CI: 1.61 - 2.97), particularly with ertugliflozin, sotagliflozin, and dapagliflozin.

CONCLUSION:

SGLT2 inhibitors confer renal protection in both diabetic and non-diabetic populations, supporting their use in CKD management across a broad spectrum of patients. However, careful drug selection is warranted in diabetic patients at risk for DKA.

Exploring SGLT-2 inhibitors and sarcopenia in FAERS: a post-marketing surveillance study

BACKGROUND:

The sodium-dependent glucose transporters 2 inhibitors (SGLT-2i) is associated with body weight loss but the composition of the losing weight remains unclear.

RESEARCH DESIGN AND METHODS:

Disproportionality analyses, including the reporting odds ratio (ROR), the proportional reporting ratio (PRR), the Bayesian confidence propagation neural network (BCPNN), and the multi- item gamma Poisson shrinker (MGPS) algorithms, were employed to quantify the signals of SGLT-2i-associated musculoskeletal and connective tissue disorders AEs.

RESULTS:

The search retrieved a total of 3,206 cases of musculoskeletal and connective tissue disorder-related AEs during the reporting period. This included 1,061 cases for Canagliflozin, 1,052 cases for Dapagliflozin, 1,074 cases for Empagliflozin, and 19 cases for Ertugliflozin. Fifteen preferred terms (PTs) with significant disproportionality were retained. No musculoskeletal and connective tissue system-related AE signals were reported for Ertugliflozin. We identified a risk of muscle necrosis with Canagliflozin use, a risk of sarcopenia with Dapagliflozin use, and a chance of muscle atrophy with Dapagliflozin and Empagliflozin prescriptions. Most cases occurred within the first month after SGLT-2i initiation, and AEs can persist beyond 360 days of use.

CONCLUSIONS:

Our study identified potential new musculoskeletal and connective tissue disorder-related AE signals associated with SGLT-2 inhibitors.

Introduction::

The goal of this study was to find out how SGLT2 inhibitors and GLP-1 receptor agonists help persons with Type 2 Diabetes Mellitus (T2DM) keep their blood sugar levels stable.

Methodology::

There were 120 people in the trial. They were randomly given either empagliflozin, Canagliflozin, Dapagliflozin, Ertugliflozin, Bexagliflozin, Sotagliflozin (an SGLT2 inhibitor) or liraglutide, Sitagliptin, Saxagliptin, Linagliptin, Alogliptin, Vildagliptin (n GLP-1 receptor agonist) to consume for 24 weeks. The major goal was to lower the levels of HbA1c. Some of the secondary outcomes included losing weight, markers of heart health, and adverse events.

Results::

Both treatments brought down HbA1c levels a lot, although GLP-1 agonists brought them down a little bit more. Both groups’ systolic blood pressure and LDL cholesterol levels improved, but the group that took GLP-1 agonists lost more weight. People who took SGLT2 inhibitors didn’t have as many stomach problems, whereas people who took GLP-1 agonists got nauseated and threw up.

Conclusion::

The study showed that both types of medicines do a decent job of keeping blood sugar levels steady. But GLP-1 agonists are better at helping people lose weight and keep their hearts healthy, so they are also a fantastic choice for people who have these problems. It’s crucial to consider what each patient needs and what could go wrong while establishing treatment plans for them.

Layoffs in Pharma Giants: Major Executive Changes at BMS, Merck, Johnson & Johnson

In November, the pharmaceutical industry experienced significant changes in executive personnel, with adjustments at several well-known domestic and international enterprises drawing widespread attention. These changes not only reveal internal strategic adjustments but also signal new trends for the industry's future development. Bristol-Myers Squibb (BMS) recently announced further layoffs as part of its cost-saving strategy. According to the New Jersey Worker Adjustment and Retraining Notification (WARN) document, this round of layoffs affects 195 employees.
