Bisphenol A bis (diphenyl phosphate) disrupts tryptophan metabolism through microbiota dysbiosis to promote intestinal toxicity

Organophosphorus flame retardants (OPFRs) are associated with intestinal injury. Bisphenol A bis(diphenyl phosphate) (BDP), an emerging OPFR that presents widely in organisms and humans, may induce intestinal toxicity, yet the effect and underlying mechanism remains unclear. In this study, zebrafish were exposed to BDP at 2, 20 and 200 μg/L for 21 days. Distinct histopathological changes in the intestine of zebrafish were observed, and the relative expressions of mucus secretion and tight junction related genes (MUC-2, Occuludin a and ZO-1) were all downregulated. Through the integrated analysis combining metabolomics and metagenomics, the results demonstrated that BDP exposure downregulated the abundances of microbiota Peptostreptococcus, Clostridium, Bombilactobacillus and Sporolactobacillus in zebrafish intestines, to depress tryptophan metabolism and eventually reduce the abundances of tryptophan metabolites. As a result, the expression of AhR, an important receptor activated by tryptophan metabolites, was inhibited to downregulate IL-22 expression, promoting intestinal toxicity. In vivo experiment with indole-3-propionic acid supplement alleviated the pathological changes, which further confirmed that BDP destroyed microbiota-tryptophan metabolism homeostasis to interfere with the AhR-IL-22 axis, eventually promoted pathological toxicity in the intestines. This study highlights vulnerability of intestines to BDP, and provides first insight into the mechanism through which BDP threats intestinal health.

BDP/FF/GB single-inhaler triple therapy in COPD: real-world effectiveness, safety, and adherence in a pooled analysis of 5,000 patients

BACKGROUND:

Chronic Obstructive Pulmonary Disease (COPD) represents a major global health challenge, frequently necessitating triple therapy for patients unresponsive to dual bronchodilation. Single-inhaler triple therapy (SITT) combining beclomethasone dipropionate/formoterol fumarate/glycopyrronium bromide (BDP/FF/GB) offers a practical and effective solution with potential adherence benefits in real-world settings.

METHODS:

This pooled analysis included patient-level data from six European prospective observational studies assessing the real-world effectiveness and safety of BDP/FF/GB SITT in COPD patients over 6 months. Outcomes included changes in health-related quality of life (HRQoL; measured by COPD Assessment Test [CAT] scores), lung function (FEV₁), and adherence, with safety and exacerbation rates assessed through 12 months. Subgroup comparisons based on prior therapy, exacerbation history, and disease severity were also performed.

RESULTS:

Among 5523 patients enrolled, 4541 were included in the full analysis set. Significant improvements in HRQoL were observed, with mean CAT score reductions of -3.8 and -4.7 at months 3 and 6, respectively (p < 0.0001). Patients with at least one exacerbation decreased from 93.1% (pre-treatment) to 35.7% at 12 months (p < 0.0001). FEV₁ improvements were observed up to 6 months, with over 40% of patients achieving ≥100 mL increase. Adherence significantly improved, with poor-adherence patients transitioning to higher categories over time. Logistic regression identified poor baseline adherence (vs. good), ex-smokers (vs non-smokers), and experiencing two (vs. >2) exacerbations as predictors of treatment response. Adverse events were reported by 21.9% of patients.

CONCLUSION:

Extrafine BDP/FF/GB SITT demonstrated significant real-world safety and effectiveness in improving symptoms, lung function, and adherence.

Improved lung function with beclomethasone/formoterol versus beclomethasone alone in asthma: the FORCE2 study

OBJECTIVE:

This study aimed to confirm the efficacy and safety of the inhaled fixed-dose combination of beclomethasone dipropionate (BDP) plus formoterol fumarate (FF) vs. BDP in patients with asthma.

METHODS:

After a two-week run-in period with asthma maintenance therapy switched to BDP via pressurized metered-dose inhaler (pMDI), eligible patients were randomized to BDP/FF or BDP, both via pMDI, for 12 wk. The primary objective was to demonstrate superiority of BDP/FF over BDP for change from baseline at Week 12 in area under the curve between 0 and 12 h post-dose (AUC_0-12h) of forced expiratory volume in 1 sec (FEV₁). The key secondary objective was to demonstrate superiority of BDP/FF over BDP for change from baseline at Week 12 in peak FEV₁ within the first 3 h post-dose. Safety and tolerability were assessed as secondary endpoints.

RESULTS:

Of 576 patients randomized to treatment, 543 completed the study (BDP/FF: 276/287 [96.2%]; BDP: 267/289 [92.4%]). The primary and the key secondary objectives were met, with BDP/FF vs. BDP adjusted mean differences of 104 (95% confidence interval 61, 148) mL and 124 (76, 173) mL for FEV₁ AUC_0-12h and peak FEV₁ at Week 12, respectively (p < 0.001 for both). A similar proportion of patients experienced adverse events in the two treatment groups (26.9% vs. 26.4%), with most events mild or moderate in severity and not considered related to study drug.

CONCLUSIONS:

The study met its aims, demonstrating the contribution of FF to BDP in lung function improvement, with both treatments being well tolerated.ClinicalTrials.gov NCT05292586.

Govt bans 156 medicines including antibiotics, painkillers, multivitamins: Here is the full list

New Delhi: In a significant move aimed at safeguarding public health, the Indian government has banned 156 fixed-dose combination (FDC) drugs, including a variety of antibiotics, painkillers, multivitamins, and medications for treating fever and hypertension. This decision marks the largest crackdown on FDCs since 2016, when 344 such drugs were prohibited. The Union Ministry of Health and Family Welfare issued a gazette notification on Thursday, officially prohibiting the manufacture, sale, and distribution of the banned FDCs. The ban was based on the recommendations of an expert panel that reviewed a total of 324 FDCs. Impact on Pharmaceutical Industry The banned FDCs include popular combinations such as mefenamic acid and paracetamol injections, commonly used for pain relief and fever, and omeprazole magnesium with dicyclomine HCl, used to treat abdominal pain. The ban is expected to impact major pharmaceutical companies, including Sun Pharmaceuticals, Cipla, Dr. Reddy's Laboratories, Torrent Pharmaceuticals, and Alkem Laboratories.
