This study is an open, single-arm, single-center phase II clinical study, which aims to evaluate the efficacy and safety of adebelizumab combined with irinotecan liposome in the second-line treatment of advanced small cell lung cancer, explore biomarkers to predict the efficacy, and further select the dominant population in immunotherapy.
In this study, 46 patients with extensive SCLC who had previously received immunotherapy combined with chemotherapy were enrolled. Subjects who met the enrollment conditions will be given adebelizumab combined with irinotecan liposome at the same time. After 4-6 cycles of treatment, if their condition has not progressed, they will be treated with adebelizumab until the disease progresses, toxicity can not be tolerated or other reasons specified in the scheme can be terminated. Subjects who finish the treatment will enter the follow-up period for safety follow-up and survival follow-up.

Neoadjuvant Low-Dose Radiotherapy Combined With Adebrelimab and Single-Agent Albumin-Bound Paclitaxel Chemotherapy for Early-Stage Oral Squamous Cell Carcinoma: A Prospective, Single-Arm Clinical Trial

This is a prospective, single-arm, Phase II clinical trial. The included population consisted of untreated patients with histologically confirmed stage I-II oral squamous cell carcinoma. Eligible patients received neoadjuvant low-dose radiotherapy combined with adibelizumab and monotherapy with albumin paclitaxel for two cycles. Radical surgical resection is planned to be carried out 3 to 4 weeks after neoadjuvant therapy. Patients who obtain PCR after evaluation by the researchers after the operation will be maintained for 6 cycles of immunomonotherapy. The primary endpoints were the rate of pathological response and the rate of exemption from cervical lymph node dissection. The secondary endpoints included progression-free survival, local control rate, distant metastasis-free rate, overall survival, safety and quality of life of patients, as well as the exploration of biomarkers

BACKGROUND:

Small cell lung cancer (SCLC) is initially highly sensitive to chemotherapy, which often leads to significant tumor reduction. However, the majority of patients eventually develop resistance, and the disease is further complicated by its "cold" tumor microenvironment, characterized by low tumor immunogenicity and limited CD8+ T cell infiltration. These factors contribute to the poor response to immunotherapy in many cases of extensive-stage SCLC (ES-SCLC). High-dose chemotherapy has shown potential in enhancing tumor cytoreduction, but its use is often limited by severe hematologic toxicity. Combining chemotherapy with immune checkpoint inhibitors (ICIs) can create a synergistic effect by promoting immunogenic cell death and enhancing immune activation. Autologous hematopoietic stem cell transplantation (auto-HSCT) provides a means to support hematopoietic recovery, mitigate chemotherapy-induced myelosuppression, and contribute to immune reconstitution. In this context, the integration of auto-HSCT with dose-intensified chemotherapy and ICIs aims to both protect the bone marrow and enhance antitumor immune responses, potentially overcoming resistance to immunotherapy in ES-SCLC.

METHODS:

A phase I, single-center, single-arm trial was designed to evaluate the safety and efficacy of auto-HSCT-supported dose-intensified chemotherapy combined with adebrelimab in treatment-naive ES-SCLC patients. Participants will receive induction chemotherapy followed by stem cell mobilization, apheresis, and cryopreservation. After successful mobilization, consolidation chemotherapy with stem cell reinfusion and granulocyte colony-stimulating factor (G-CSF) support will be performed. Maintenance therapy with adebrelimab continues until disease progression or unacceptable toxicity. Safety and efficacy data, including adverse events, objective response rate (ORR), progression-free survival (PFS), and overall survival (OS), will be analyzed.

RESULTS:

The study aims to enhance treatment outcomes by overcoming resistance to immuno-chemotherapy and promoting immune reconstitution. The trial is ongoing at the First Affiliated Hospital of Guangzhou Medical University.

A prospective, phase II, neoadjuvant study based on chemotherapy sensitivity in HR+/HER2‐ breast cancer‐FINEST study

Abstract:

Background:

Hormone receptor‐positive (HR+)/humaal growth factor receptor 2‐negative (HER2‐) breast cancer, the most common breast cancer type, has variable prognosis and high recurrence risk. Neoadjuvant therapy is recommended for median‐high risk HR+/HER2‐ patients. This phase II, single‐arm, prospective study aimed to explore appropriate neoadjuvant treatment strategies for HR+/HER2‐ breast cancer patients.

Methods:

Eligible female patients with newly diagnosed, untreated HR+/HER2‐ breast cancer received 2 cycles of nab‐paclitaxel and carboplatin (nabPCb). Magnetic resonance imaging (MRI) was performed to assess tumor responses, and 40% regression of the maximal tumor diameter was deemed chemo‐sensitive. Chemo‐sensitive patients continued nabPCb for 4 more cycles (group A). Chemo‐insensitive patients were randomized to groups B, C, and D at a ratio of 1:3:1 to receive a new chemotherapy for 4 cycles or endocrine‐immune‐based therapy (dalpiciclib, letrozole and adebrelimab, with goserelin if patients were premenopausal) for 4 cycles or to undergo surgery. Peripheral blood and core‐needle biopsy (CNB) samples were collected before treatment, followed by a next‐generation sequencing (NGS) panel detection and similarity network fusion (SNF) typing through digital pathology data. The primary endpoint was the pathological complete response (pCR) rate, and the secondary endpoint was the clinical objective response rate (ORR).

Results:

A total of 121 patients were enrolled (67.8% with stage III disease), with 76, 9, 27, and 9 patients in groups A, B, C and D, respectively. The total pCR rate was 4.1%, and all patients who received pCR were in group A. Group C had a better ORR than Group B (81.5% vs. 66.7%). Exploratory analysis revealed that patients with the SNF4 subtype were the most sensitive to nabPCb (pCR rate of 21.1% vs. 1.8% in group A), whereas patients in group C with the SNF2 subtype were more sensitive to endocrine‐immune‐based therapy (Miller‐Payne grade 4‐5, 45.5% vs. 6.3%).

Conclusions:

Converting to endocrine‐immune‐based therapy improved the ORR, but not the pCR rate in chemo‐insensitive patients. Neoadjuvant chemotherapy and endocrine therapy are not mutually exclusive. The SNF4 subtype of HR+/HER2‐ breast cancer was more chemo‐sensitive, whereas the SNF2 subtype might be more sensitive to immunotherapy.

[18F]FDG PET/CT for predicting neoadjuvant PD-L1 blockade monotherapy treatment response in patients with locally advanced esophageal squamous cell carcinoma: a preliminary study

PURPOSE:

To investigate the predictive value of 2-[18F]-fluoro-2-deoxy-D-glucose ([¹⁸F]FDG) PET/CT for evaluating primary tumor (PT) and lymph node (LN) responses after neoadjuvant programmed death-ligand 1 (PD-L1) blockade monotherapy in patients with locally advanced esophageal squamous cell carcinoma (LA-ESCC).

METHODS:

In the single-arm phase 1b NATION-1907 trial (NCT04215471), 23 patients with LA-ESCC received two cycles of neoadjuvant PD-L1 blockade Adebrelimab followed by surgery. Among these, 18 patients underwent [¹⁸F]FDG PET/CT scans both before immunotherapy and prior to surgery. Standardized uptake value corrected for lean body mass (SUL)-derived parameters, including SUL_max and SUL_peak, were documented for PTs and LNs. Lesions > 1cm³ were segmented using thresholds of 41% and 50% of SUL_max, respectively, following European Association of Nuclear Medicine (EANM) guidelines, with metabolic tumor volume (MTV) and total lesion glycolysis (TLG) calculated. Percentage changes of all metabolic parameters were also recorded. Residual viable tumor ≤ 33% were classified as well-responders, whereas residual viable tumor > 33% were classified as poor-responders based on histological evaluation.

RESULTS:

In the PT analysis, 10 patients were classified as PT well-responders and 8 as PT poor-responders. All post-treatment metabolic parameters, except MTV, were significantly lower in well-responders compared to poor-responders. The %ΔMTV, %ΔTLG were significantly higher in the poor-responder group (all P < 0.05). ROC curves indicated %ΔMTV₄₁ exhibited optimum performance in predicting well-responders, with an AUC of 0.875 (cut-off: -31.01). Furthermore, %ΔMTV₄₁ significantly predicted patients' recurrence-free survival (RFS) (P < 0.1). In the LN analysis, 7 LNs were classified as well-responders and 10 as poor-responders. Pre-treatment SUL_max, SUL_peak were significantly lower in poor-responders compared to well-responders. Post-treatment MTV₅₀ and all percentage changes in parameters were significantly higher in the poor-responder group (all P < 0.05). Receiver operating characteristic curve (ROC) analysis indicated %ΔTLG₅₀ had excellent predictive performance for well-responders, with an AUC of 1.000 (cut-off: -7.5). However, there was no significant correlation between the metabolic response evaluations for PTs and LNs.

CONCLUSION:

The metabolic parameters of [¹⁸F]FDG PET/CT, particularly %ΔMTV and %ΔTLG, could effectively predict well-responders among both PTs and LNs to neoadjuvant PD-L1 blockade monotherapy in LA-ESCC, which may facilitate personalized immunotherapy and serve as a stratification tool in future larger-scale studies.

The First Phase 1b Trial Shows Promising Results of Neoadjuvant Adebrelimab for Esophageal Squamous Cell Carcinoma

Esophageal squamous cell carcinomaThe Chinese Academy of Sciences' Center for Systems Biology recently published a research article in Nature Medicine titled "Neoadjuvant Adebrelimab in Locally Advanced Resectable Esophageal Squamous Cell Carcinoma: A Phase 1b Trial," which reports on the results of the world’s first-ever neoadjuvant mono-immunotherapy Phase 1b clinical trial of locally advanced esophageal squamous cell carcinoma. The study proposes new strategies for predicting tumor immunotherapy efficacy based on multi-omics studies. Neoadjuvant immunotherapyEsophageal cancer is one of the most prevalent malignant tumors of the digestive tract worldwide, with over 90% of cases comprising esophageal squamous cell carcinomas (ESCC), representing nearly 60% of new cases globally. It ranks fourth among common cancers in terms of mortality. Unfortunately, most patients with esophageal cancer are in the locally advanced stage at the time of diagnosis. The current standard clinical treatment methods are neoadjuvant chemoradiotherapy or chemotherapy after surgery. However, these treatments have limited efficacy, leading to local recurrence or distant metastasis and ultimately compromising patient survival rates. Recent years have seen the emergence of immunotherapy, represented by checkpoint inhibitors, which has demonstrated positive therapeutic effects in advanced solid tumors. Its application has gradually extended from second-line maintenance treatment to neoadjuvant treatment. However, the safety and benefits of neoadjuvant mono-immunotherapy for esophageal squamous cell carcinoma have not yet been reported, and there is no clear biological marker for predicting the efficacy of such immunotherapy. To explore the safety and effectiveness of neoadjuvant mono-immunotherapy for patients with locally advanced resectable esophageal squamous cell carcinoma, the researchers carried out a prospective, single-center, single-arm, nonrandomized Phase 1b clinical trial (NATION-1907, registered number NCT04215471). NATION-1907 study design30 patients with stage II-IV resectable esophageal squamous cell carcinoma who underwent surgery after neoadjuvant treatment with Adebrelimab (anti-PD-L1 antibody) were enrolled for the study. Based on assessments of safety and effectiveness, the rate of major pathological response (MPR, tumor shrinkage ≥90%) and overall survival (OS) were elicited. Tumor tissues and blood samples of enrolled patients before and after treatment were collected to integrate genome and transcriptome sequencing, TCR sequencing, and multiple immunofluorescence techniques. The study compared the different molecular characteristics of the sensitive and insensitive groups to explore the potential mechanisms of treatment response and resistance. Safety, efficacy and biomarkersIn the safety evaluation of the study, no adverse reactions over Grade 3 were observed, and 24% of patients achieved major pathological remission (MPR), with 2-year overall survival (OS) at 92% and 2-year disease-free survival (RFS) at 100%. Both treatment adverse reactions and 2-year survival status assessment demonstrated that the neoadjuvant anti-PD-L1 monotherapy adopted in this study was significantly better than neoadjuvant chemotherapy or chemoradiotherapy (compared with CMISG1701 clinical study), showing good safety and efficacy of the treatment regimen. Preliminary efficacyTo compare well responders and poor responders, the research team further analyzed potential biomarkers associated with efficacy stratification. Although no significantly correlated mutation features were identified at the genome variation level, the researchers constructed an "IFN/EMT score" consisting of 12 characteristic genes based on transcriptome data. The score could significantly distinguish patients in different treatment response groups. Further integrative analysis of immune cell composition and enriched cancer-related pathways defined three immune microenvironment phenotypes: immune-enriched, tumor-proliferation, and fibroblast-enriched. Immune-enriched patients exhibited more significant tumor shrinkage compared to the other two types, and the response group had a higher proportion of immune-enriched patients. Immune-enriched TME phenotype in responsive patients.The analysis of non-responsive patients found that their tumors harbored higher densities of immunosuppressive cells such as M2 macrophages and fibroblasts. These patients may require a combination of immune-suppressing strategies to benefit from immunotherapy. Using multiple immunofluorescence staining techniques, the research team further characterized the spatial distribution of the microenvironment in 2 non-responsive patients, classified as immune-excluded and immune-suppressive. In immune-excluded tumors, immune cells accumulated at the tumor margin with limited infiltration into the core, while the immune-suppressive tumor core accumulated more pro-tumor macrophages and fewer B cells. By analyzing the immune repertoire (TCR-seq) of tumor tissues and peripheral blood mononuclear cells (PBMCs), the researchers found that T cell receptor (TCR) diversity in PBMCs and the clonality of tumor-infiltrating T cells (TILs) were positively correlated with treatment response. This suggests that the degree of immune activation in peripheral blood and the infiltration of T cells into the tumor microenvironment may play important roles in predicting the efficacy of immunotherapy.
