A Phase 1 Study to Evaluate the Safety and Tolerability of Escalating Doses of VAS-101 in Subjects With Stable Sickle Cell Disease

Background:
Sickle cell disease (SCD) is an inherited blood disorder. The disease affects the ability of red blood cells to carry oxygen. Research has shown that curcumin, a natural compound found in turmeric, can improve the health of red blood cells in people with SCD. But the body cannot absorb curcumin well when it is taken by mouth. Researchers want to know if a skin gel (VAS-101) can help the body better absorb curcumin. VAS-101 contains curcumin, which comes from turmeric.
Objective:
To test VAS-101 in people with stable SCD.
Eligibility:
People aged 18 to 70 years with stable SCD.
Design:
People who want to join the study will be screened with physical exam with blood tests to see if they are eligible. If they qualify, they can enroll in the study.
Participants will have up to 15 clinic visits over about 14 weeks. Some may need to stay overnight in the hospital for up to 2 days to make it easier to collect blood samples after the gel is applied.
For 6 weeks, a special gel called VAS-101 will be put on the forearms in the clinic two times a week. Staff will rub the gel into the skin for at least 30 seconds using a soft toothbrush. The area stays uncovered for at least 10 minutes, then is covered with a bandage or sleeve. After 24 hours, the dressing can be removed and the skin can be washed.
Some visits will include blood tests and other exams. On three visits, a test called near infrared spectroscopy (NIRS) will be done. For this test, probes are placed on the skin to measure blood flow, oxygen levels, and the makeup of skin and muscle. A blood pressure cuff is used to squeeze the arm for up to 5 minutes.
The last clinic visit will happen about 4 weeks after the final gel application....

CONTEXT:

Curcumin-containing dietary supplements are widely marketed with claims of enhanced bioavailability, despite well-recognized limitations related to poor aqueous solubility, chemical instability, and extensive first-pass metabolism. Comparisons among commercially available products using physiologically relevant performance metrics remain limited.

OBJECTIVE:

To systematically evaluate disintegration, dissolution, and bioaccessibility of curcuminoids and (S)-ar-turmerone from a cross section of commercially available turmeric dietary supplements under fasted- and fed-state biorelevant conditions.

MATERIALS AND METHODS:

Eight marketed turmeric supplements representing diverse formulation strategies were assessed for disintegration and dissolution using USP-aligned methods in fasted- and fed-state simulated gastric and intestinal media (FaSSGF, FaSSIF, FeSSGF, FeSSIF). Bioaccessible concentrations, quantities, and dose fractions of curcuminoids and (S)-ar-turmerone were quantified after 3 h.

RESULTS:

All products exhibited poor dissolution overall, with no formulation achieving greater than 40% total release. Dissolution was lowest under fasted-state conditions and improved modestly in fed-state gastric media, reflecting the influence of lipid content. Products with higher curcuminoid loads per capsule generated greater absolute bioaccessible concentrations despite poor release efficiency, whereas a phytosome formulation achieved superior release despite a lower dose. Several products marketed as "enhanced" formulations demonstrated poor disintegration and low bioaccessibility.

DISCUSSION:

These findings indicate that bioaccessible concentration is governed jointly by dosage-form performance and curcuminoid dose loading, and that plasma exposure metrics dominated by conjugated metabolites may not reliably reflect formulation performance.

CONCLUSION:

Commercial turmeric supplements exhibit substantial limitations in biorelevant disintegration and dissolution. Superior in vitro release is a prerequisite - but not a guarantee - for enhanced systemic exposure, underscoring the need for cautious interpretation of bioavailability claims.

Low, plasma level‑informed native curcumin concentrations fail to induce cell death in human lung and colorectal cancer cells

BACKGROUND/OBJECTIVES:

Curcumin, a dietary polyphenol derived from turmeric, has been widely studied for its anti-cancer properties, yet its effects at clinically relevant concentrations remain unclear. This study investigates the anti-cancer effects of curcumin at low in vitro concentrations selected based on reported plasma ranges using in vitro lung and CRC models, with a focus on underlying cellular mechanisms.

METHODS:

Curcumin was tested at 4, 10, 20, and 50 µg/mL in two CRC cell lines (Caco-2 and HT29) and two lung cancer cell lines (A549 and H460).

RESULTS:

MTS assays showed that at a low concentration of Curcumin 4 µg/mL, cell viability remained above 100% across all cell lines (A549: 102.1%, H460: 101.1%, Caco-2: 103.6%, HT29: 104.9%, n = 3, p > 0.05) and had no significant effect on cell death. Immunofluorescence analysis showed increased nuclear Cyclin D1 levels at 4 µg/mL curcumin in H460 and HT29 cells (p < 0.001), and no change in Caco-2 cells (p = 0.17), and a significant reduction in A549 cells (p < 0.001), suggesting promotion of cell cycle progression in H460 and HT29 cells only. Western blotting analysis showed higher levels of procaspase-3 without evidence of cleavage at 4 µg/mL, indicating the absence of apoptosis. A reduction in procaspase 3 levels were observed at 20 µg/mL (Caco-2, p < 0.05) and 50 µg/mL (H460, p < 0.05; A549, and HT29, p > 0.05).

CONCLUSIONS:

These findings suggest that at low. plasma level-informed concentrations, curcumin may support cancer cell survival rather than induce cytotoxicity. This study highlights the need for further pre-clinical evaluation of polyphenols at clinically relevant concentrations.

Epigenetic regulators polyphenols in neurodegenerative diseases: a promising intervention strategy

BACKGROUND:

Neurodegenerative diseases are complex disorders characterized by the progressive loss of neuronal structure and function, involving pathological mechanisms such as oxidative stress, chronic inflammation, protein misfolding, and impaired synaptic plasticity. Recent studies have revealed that epigenetic regulation plays a critical role in the onset and progression of these diseases, including mechanisms such as DNA methylation, histone modifications, and non-coding RNA regulation. Natural polyphenolic compounds, known for their safety and multi-target properties, have emerged as promising candidates for neuroprotection and therapeutic intervention.

METHODS:

This review summarizes recent advances in the neuroprotective effects of polyphenols in neurodegenerative diseases through epigenetic mechanisms. It focuses on their regulation of DNA methyltransferase activity, histone acetylation status, and non-coding RNA expression, as well as their influence on neurotrophic factors, inflammatory mediators, and synapse-related gene expression.

RESULTS:

Polyphenolic compounds such as curcumin, resveratrol, and epigallocatechin gallate (EGCG) have been shown to modulate epigenetic enzyme activity, alleviate neuroinflammation, improve mitochondrial function, and promote neuroregeneration, demonstrating multi-level neuroprotective effects. However, clinical translation is limited by low oral bioavailability, rapid metabolism, and insufficient brain-targeting capacity. Emerging strategies including nano-delivery systems, prodrug design, and omics technologies hold promise in overcoming these limitations and enhancing therapeutic efficacy.

CONCLUSIONS:

Polyphenols exert multi-target regulatory effects via epigenetic mechanisms in neurodegenerative diseases, showing great potential for intervention. In the future, integrating precision nutrition, personalized treatment, and multi-omics approaches may facilitate the development of novel epigenetic-based strategies for the prevention and treatment of neurodegenerative disorders.

Vascarta Announces Phase I Results of VAS-101 Administered Sublingually in Sickle Cell Disease Patients

SUMMIT, N.J., June 24, 2026 /PRNewswire/ -- Vascarta Inc., a clinical-stage biopharmaceutical company advancing innovative treatments for pain and inflammation, announced today the successful completion of the sublingual arm of its Phase I clinical trial evaluating VAS-101 (Vasceptor®; topical curcumin gel) for the treatment of sickle cell disease (SCD). The study was conducted by the Foundation for Sickle Cell Disease Research (FSCDR) in Hollywood, Florida, under the supervision of the principal investigator, Dr. Gershwin Blyden. The findings were published today in the Journal of Sickle Cell Disease: Low-Dose Sublingual Curcumin Improves Red Blood Cell Sickling Kinetics in Sickle Cell Disease: Interim Results from a Pilot Study | Journal of Sickle Cell Disease | Oxford Academic Five (5) patients with sickle cell disease were treated sublingually with a minimal dosing (0.1 mL) of VAS-101 twice per week for a total of 8 treatments. Their blood was drawn and analyzed weekly. The duration of treatment was 29 days. All participants demonstrated favorable safety and tolerability. Directional improvements were observed across several patient-reported outcomes. Mean pain scores decreased from 7.0 at baseline to 6.6 by Day 15, with several participants demonstrating clinically meaningful reductions from baseline. Measures of fatigue and energy also improved, with out-of-energy scores decreasing from 3.6 at baseline to 2.2 by Day 22 and fatigue limits improving from 2.8 to 2.0. Psychosocial ePRO measures showed stability or improvement across most participants, including reductions in worry, loneliness, and depressive symptoms. Assessment of red blood cell (RBC) functional biomarkers demonstrated improvements as measured by the DSA. From baseline to Day 29, the Area Under the Curve (AUC10) decreased from 419±140 to 394±118 %*min. Morphological Point of Sickling at 50% (mPoS@50%) increased from 4.9±1.0 to 5.5±1.2 minutes, indicating a delay in sickling. Additionally, the rate of sickling decreased from 47±23 to 38±17 %/min. Collectively, these changes reflect improved sickling kinetics over the treatment period. FA-WB-VCAM and FA-WB-Psel adhesion indices (fAI) were below previously established critical thresholds (FA-WB-VCAM:400 cells/mm2, FA-WB-Psel:50 cells/mm2) predictive of future self-reported vaso-occlusive crises, VOCs in four (4) subjects at baseline and, as expected, no improvements in adhesion were seen at Day 29. Only a single participant exhibited a baseline FA-WB-VCAM fAI exceeding the clinical threshold (>502 cells/mm²). Red blood cell mechanical fragility indices (MFI) were within normal range at baseline and remained stable over the course of the study. Baseline concentrations of standard inflammatory markers were and remained below the lower limit of detection for the initial three participants; consequently, further analytical testing was suspended. VAS-101 demonstrated safety and tolerability in the sublingual cohort. Improvements in integrated ePRO and RBC functional biomarkers were noted suggesting potential mechanistic and patient-centered benefits of VAS-101 in individuals with SCD. The resulting biomarkers shift at low dose supports advancement towards dose optimization. Future studies evaluating higher dose and frequency may yield greater improvements in pain, ePRO measures, and laboratory biomarkers. Pre-selecting a cohort that exhibits a higher adhesive burden, indicative of more inflammation may show further improvements in RBC health normalization.
