SummaryTrazodone, a small molecule drug, targets the serotonin transporter and acts as a serotonin reuptake inhibitor. The goal of this clinical trial is to learn how three current insomnia therapies (trazodone, daridorexant, cognitive behavioral therapy for insomnia) compare with each other in peri- and post-menopausal women. It will also learn about the safety of the treatments. The main questions it aims to answer are:
Which commonly used insomnia therapies are most effective and safe for improving insomnia symptoms in peri- and post-menopausal people?
How well do the therapies work for people from different socio-demographic backgrounds, who are at different stages of menopause, and who have different menopause-associated conditions (e.g., sleep apnea, mood disturbance, etc.)?
What medical problems do participants have when taking part in these treatments?
Participants will:
Be asked to take trazodone every night, take daridorexant every night, or participate in a behavioral program for insomnia, for a duration of 12 months.
Participate in a total of one in-person visit and 5 virtual visits (phone calls) over the 12 months.
Wear (and keep) a Fitbit and fill out a daily sleep diary for at least 4 weeks over the 12 months.
Fill out online surveys at 5 time-points over the 12 months.

CHIM.RIUN. ANGELINI FRANCESCO ACRAF S.P.A. as the reference preparation, and to evaluate the bioequivalence of the two preparations under fasting and postprandial conditions through a single-center, randomized, open, single-dose, two-preparation, two-sequence, two-period, double-crossover clinical study. The Efficacy of Trazodone for the Treatment of Insomnia in Older Adults: A Randomized Controlled Trial (The TRADITION Study)

This study aims to evaluate how effective trazodone is in treating insomnia in adults aged 65 years and older. The main question it aims to answer is :
- Is trazodone more effective than a placebo in reducing the severity of insomnia symptoms in older adults?
Participants will :
* Take 25 to 50 mg of trazodone or a matching placebo before bed for 28 days. After a 2-week break, they will then take the other medication for another 28 days.
* Visit the clinic three times for checkup and test
* Complete a sleep diary and wear an actimeter during the night.

Effect of trazodone on heart rate, heart rate variability, and QT-intervals in dogs

INTRODUCTION/OBJECTIVES:

To determine the effect of trazodone and placebo on heart rate (HR), heart rate variability (HRV), and QT intervals (QTi) in dogs after oral administration of trazodone or placebo.

ANIMALS, MATERIALS AND METHODS:

Twenty healthy adult client-owned dogs were included in the study. Dogs were randomized in a double-blind, placebo-controlled crossover trial. Dogs received trazodone (6 mg/kg q 8 h) or placebo for 24 h, during which a 24-h ambulatory electrocardiogram (ECG) (Holter) was recorded. Diagnostic ECGs and clinician-assessed behavior scores were obtained before and after the 24-h study period. Owners also scored the dog's behavior changes during each of the 24-h study periods. Following a minimum one-week washout period, dogs received the alternate study drug, and all procedures were repeated. Linear and mixed models were used for statistical analyses.

RESULTS:

Dogs receiving trazodone had higher average HRs (P=0.035), higher minimum HRs (P<0.001), and reduced HRV parameters (P<0.001) on the Holter recordings. Electrocardiogram-derived QTi was not different between groups. Dogs receiving trazodone appeared calmer based on owner assessments (P<0.001) and behavior scores (P=0.006).

STUDY LIMITATIONS:

Blood pressure and trazodone blood levels were not measured, making it impossible to determine the effect of these variables on the HR and ECG findings.

CONCLUSIONS:

Trazodone increased HR and decreased HRV. Possible explanations include a previously described anticholinergic effect or a possible decrease in blood pressure causing a reflex response. No demonstrable effect on QTi was identified in this cohort of healthy dogs using standard trazodone dosing.

Development and application of a sustainable approach for the determination of 95 pharmaceutical substances and metabolites in urban wastewater by means of ultra-high-performance liquid-chromatography-tandem mass spectrometry

In the last few years, the interest in pharmaceutical drugs and their metabolites as environmental pollutants is gaining growing importance.Regular monitoring and timely actions are decisive to implement appropriate water resource management strategies and to evaluate the efficiency of traditional or innovative wastewater treatment plants (WWTPs).This study describes the development and validation of an anal. procedure using 10 mL sample volume followed by direct-injection in ultra-high-performance liquid chromatog.-tandem mass spectrometry (UHPLC-MS/MS) for the simultaneous determination of 95 pharmaceutical drugs and 10 of their main metabolites in wastewater.Adequate sensitivity was recorded for all target analytes, with limits of detection below 5 ng/L for 60 out of 95 analytes and recoveries exceeding 80% for all the analytes under study.A total of 42 target analytes were detected in almost all sites, and limited differences were observed among several pharmaceutical drug arrays found at different sampling sites.In addition, an abatement yield higher than 50% was observed for only 12 of the 42 detected substances.The procedure, which combined direct injection and small sample volume collection, showed great potential and efficiency for the high-performance determination of pharmaceutical drugs in wastewater.

Analysis of factors affecting laboratory alert level of trazodone based on therapeutic drug monitoring data

AIMS:

Previous studies have revealed the importance of therapeutic drug monitoring for personalized trazodone dosing. However, studies on risk factors related to trazodone plasma concentrations that exceed laboratory alert levels are lacking. This study aimed to identify the factors influencing the trazodone plasma concentration-to-dose (C/D) ratio in the Chinese population. Additionally, it attempted to comprehensively investigate the factors contributing to trazodone concentrations exceeding 1200 ng/mL.

METHODS:

A single-center, retrospective, cross-sectional study was conducted at Xi'an Mental Health Center in China, where data on Trazodone-treated patients between 2020 and 2023 were collected.

RESULTS:

The C/D ratios of trazodone were significantly influenced by age, the formulations of trazodone, and the concomitant use of fluvoxamine. Additionally, the results revealed that 21.67 % of patients receiving daily trazodone doses of 25-200 mg demonstrated plasma concentrations exceeding laboratory alert levels. A logistic regression analysis using a enter method determined patient older age [1.04 (95 CI: 1.01-1.07)], higher daily dose [1.04 (95 CI: 1.02-1.05)], higher total protein level [1.10 (95 CI: 1.02-1.19)] and concomitant use of fluvoxamine [7.99 (95 CI: 2.48-25.72)] as risk factors for surpassing laboratory alert levels in plasma concentrations.

CONCLUSION:
