Direct oral anticoagulants versus phenprocoumon: Mortality, thromboembolism and major hemorrhage. A systematic review and meta-analysis of routine data studies

BACKGROUND:

In the randomized controlled trials (RCTs) of the non-vitamin K dependent oral anticoagulants (NOACs) that led to their approval, the vitamin K antagonist (VKA) warfarin was used as a comparator drug. The efficacy and safety of NOACs compared to phenprocoumon, the VKA predominantly used in Germany, are unknown, as it has not been studied in RCTs.

METHODS:

For this systematic review and meta-analysis (registration: CRD42024619047), we systematically searched 3 databases for studies based on routine data that yielded matched or adjusted results for overall mortality, thromboembolism, and major hemorrhage in anticoagulant-naïve patients who were treated with either phenprocoumon or a NOAC.

RESULTS:

Seven studies based on German routine data were identified, covering a total of 1 842 015 anticoagulant-naïve patients. In a pooled analysis of all NOACs (apixaban, dabigatran, edoxaban, and rivaroxaban), these were associated with a notably higher risk of mortality (hazard ratio [HR] 1.14, 95% confidence interval [1.00; 1.30]), a higher risk of thromboembolic events (HR 1.08, [1.01; 1.15]), and a lower risk of major hemorrhages (HR 0.80, [0.72; 0.90]) than phenprocoumon. Among individual NOACs, rivaroxaban was associated with a notably higher risk of mortality than phenprocoumon; the other NOACs also displayed a higher risk of mortality than phenprocoumon, but the differences were not statistically significant.

CONCLUSION:

In this meta-analysis of NOACs versus phenprocoumon, the former were found in a pooled analysis to be associated with a higher overall risk of mortality and a higher risk of thromboembolic events on the one hand, but a lower risk of major hemorrhages on the other hand. Warfarin was the comparator drug in all of the clinical trials that led to the approval of NOAC. The findings of this meta-analysis cast doubt on the benefit of the preferential use of NOACs in countries where phenprocoumon is the standard VKA.

Total Clinical Event Burden With Edoxaban Monotherapy Versus Dual Antithrombotic Therapy in Atrial Fibrillation and Stable Coronary Artery Disease: Insights From the EPIC‐CAD Trial

Background:

Oral anticoagulation is essential for stroke prevention in atrial fibrillation, whereas antiplatelet therapy remains standard for coronary artery disease (CAD), raising concerns about bleeding when both are combined. The effect of these regimens on recurrent events, reflecting cumulative disease burden, has not been fully evaluated.

Methods:

This post hoc analysis of the EPIC‐CAD (Edoxaban Versus Edoxaban With Antiplatelet Agent in Patients With Atrial Fibrillation and Chronic Stable Coronary Artery Disease) trial included 1040 patients with atrial fibrillation and stable CAD randomized to edoxaban monotherapy or edoxaban plus a single antiplatelet agent. The primary end point was the total number of net adverse clinical events at 12 months.

Results:

Over a median follow‐up of 12.1 months, 155 total events occurred (123 first, 32 subsequent). Event rates were lower with edoxaban monotherapy than with dual therapy (9.4 versus 19.8 per 100 person‐years; incidence rate ratio, 0.47 [95% CI, 0.34–0.67]). Monotherapy reduced first events (incidence rate ratio, 0.46 [95% CI, 0.32–0.68]) and showed a lower cumulative hazard of total events (hazard ratio, 0.48 [95% CI, 0.32–0.71]; P <0.001). Bleeding was the predominant event type in both groups but occurred more often with dual therapy (85.7% versus 55.3%; P <0.001). Rates of ischemic events and death were similar between groups.

Conclusions:

Edoxaban monotherapy significantly reduced the overall burden of clinical events compared with dual antithrombotic therapy in patients with atrial fibrillation and stable CAD. Bleeding complications were more frequent than ischemic events, emphasizing the need to minimize bleeding risk in this population.

Associations of acute kidney injury with oral anticoagulants: a disproportionality analysis of the Food and Drug Administration Adverse Event Reporting System (FAERS) database

OBJECTIVE:

The FDA Adverse Event Reporting System (FAERS) was used to evaluate the associations between oral anticoagulants (OACs) and acute kidney injury (AKI).

METHODS:

Disproportionality analysis was applied to data in the FAERS database from January 2004 to December 2023 to detect adverse events (AEs) for various OACs. The adjusted reporting odds ratios (RORs) calculated using multiple logistic regression were used to explore the risk factors for OACs-associated AKI.

RESULTS:

The crude RORs for the associations of AKI with warfarin, rivaroxaban, dabigatran, apixaban, and edoxaban were 1.35, 2.14, 2.98, 1.33, and 3.56, respectively. The risk of OACs-associated AKI was affected by age and sex, being higher in those aged 65 years and males. The adjusted RORs for rivaroxaban, dabigatran, apixaban, and edoxaban were 1.26, 1.67, 0.65, and 2.06, respectively.Nearly 50% of AKI cases occurred within the first 2 months, and each OAC was of the early-failure type. Hospitalization and mortality rates for AKI patients after OACs were 45.53% and 22.98%, respectively.

CONCLUSIONS:

The study revealed a strong association between AKI and OACs, emphasizing the need for regular renal function monitoring , especially in elderly male patients. Further in-depth research is needed to confirm these exploratory findings.

Foreign drugmaker caught faking doctors’ petition to evade China’s price cut scheme

Amid an ongoing expansion of China’s volume-based procurement (VBP) scheme to lower drug costs, the Chinese government has singled out a fraud case involving a foreign originator drug in a warning to the industry. In an attempt to influence whether a drug gets selected for price bidding, a foreign drug innovator faked doctors’ signatures in a petition, China’s National Healthcare Security Administration (NHSA) said in a post (Chinese) on Tuesday. Among the 78 signatures from 31 hospitals, over 80% were found to be doctored. Some signatures were forged by sales representatives, some used misappropriated images from other sources, and some pointed to individuals that do not exist, according to the agency. As for the few signatures that the experts themselves indeed wrote, the doctors were mostly tricked into signing the document under the guise of survey questionnaires or post-sales follow-up, the agency said. The drug has since been included in the latest VBP batch of 65 drug varieties, which together represent a total market value of nearly 60 billion Chinese yuan ($8.8 billion), according to the NHSA. The VBP, now in its 12th round, is a government-led drug-buying program targeting off-patent medicines. Under the program, generics and the originators offer price cuts to secure procurement contracts with hospitals. The government unveiled the latest list on June 23 and is now in the bidding process. In a statement to local news outlet The Paper , Novartis confirmed it was not the unnamed drugmaker flagged in the NHSA alert. The Swiss pharma’s blockbuster heart failure treatment Entresto (sacubitril/valsartan) is one of the biggest products in this VBP round. According to local reports, Entresto and its generics generated more than 6 billion yuan of end-market sales ($880 million) at public hospitals in China last year, with the Novartis originator accounting for more than 90%. The VBP program now threatens to split up that huge market by rewarding significant shares to companies that offer lower prices. In what’s slated to be an über-competitive bidding process, more than 30 Entresto generics competitors are vying for a place under VBP, according to local drug intelligence database DXY Insight. As part of the largest patent expiry in Novartis’ history, Entresto lost its U.S. market exclusivity last year and has seen major sales declines since. Some uncertainties remain for VBP’s potential impact on Entresto. The Novartis drug is among eight products that carry an asterisk, which indicates ongoing patent disputes. A few days before the revelation of the latest VBP list last month, China’s National Intellectual Property Administration doled out a five-year patent-term extension to Entresto to make up for time lost during its drug regulatory review process, effectively pushing back the expiration of its core patent to November 2031. In a new rule introduced into VBP this time, generic rivals to those asterisked drugs must submit a declaration of non-infringement or be removed from procurement platforms. Takeda’s angiotensin II receptor blocker Edarbi (azilsartan medoxomil) and acid blocker Takecab (vonoprazan fumarate), Eli Lilly’s JAK inhibitor Olumiant, Daiichi Sankyo’s factor Xa inhibitor edoxaban and AstraZeneca’s Xigduo XR (dapagliflozin and metformin extended release) are also included in VBP despite ongoing patent disagreements. Takeda, Bayer and Eisai have also denied they were the company in the NHSA’s announcement, according to The Paper. Bayer’s drospirenone/ethinyl estradiol oral contraceptive, known commercially as Yaz or Yasmin, and its imaging contrast agent Ultravist (iopromide) are included for VBP bidding against generics. Eisai has anti-vertigo medication Merislon (betahistine) and anti-seizure med Fycompa (perampanel) on the list. In addition to the companies above, the 65-item roster includes brand-name products from almost all major pharmas, including Astellas, Fresenius Kabi, GSK, Johnson & Johnson, Pfizer, Merck & Co., Merck KGaA, Otsuka, Sanofi and Servier.
