Combination therapy with lorlatinib and mitogen-activated protein kinase pathway inhibition in previously treated ALK- or ROS1-rearranged lung cancer

BACKGROUND:

Anaplastic lymphoma kinase (ALK) or ROS proto-oncogene 1 (ROS1) rearranged non-small cell lung cancers (NSCLC) develop bypass resistance mechanisms that activate mitogen-activated protein kinase (MAPK) pathway signaling. We conducted a study to evaluate the ALK/ROS1 inhibitor lorlatinib combined with MAPK pathway inhibitors: binimetinib (MEK inhibitor) or TNO155 (SHP2 inhibitor).

PATIENTS AND METHODS:

This phase IB study employed a 3 + 3 design. Patients who had disease progression on ALK or ROS1 inhibitors received lorlatinib (50 mg or 75 mg daily) with binimetinib (30 mg or 45 mg BID) or TNO155 (40 mg or 50 mg daily). The primary objective of phase 1 was determining the recommended phase 2 dose for the combinations. The primary objective of the phase 2 portion was determining the objective response rate (ORR) of each combination. Secondary endpoints included safety and tolerability and progression-free survival.

RESULTS:

In this phase IB clinical trial, 17 patients received lorlatinib and binimetinib; two patients received lorlatinib with TNO155. All patients with ALK + NSCLC had received prior lorlatinib. Among 15 evaluable patients in the lorlatinib-binimetinib cohort, one (6.7%) had a partial response (duration of response: 114 days), eight (53.3%) had stable disease, and six (40%) experienced primary progression. Median progression-free survival on lorlatinib-binimetinib was 51 days (95% CI 39-107). Treatment-related adverse events associated with lorlatinib-binimetinib were primarily grade 1-2, including rash (65%), edema (47%), and lipid abnormalities (47%). One patient discontinued treatment for grade 2 retinopathy. In the lorlatinib-TNO155 arm, both patients stopped treatment due to rapid disease progression and pleural effusions, limiting efficacy evaluation. The study was terminated due to slow accrual before advancing to the phase 2 component.

CONCLUSIONS:

Regimens co-targeting the MAPK pathway and ALK or ROS1 had limited efficacy in unselected patients with lorlatinib-resistant NSCLC, underscoring the need for more effective and biomarker-informed treatment strategies.

Abstract:

The Kirsten rat sarcoma viral oncogene homolog (KRAS) gene is among the most frequently altered genes in cancer, and the KRAS protein was long deemed undruggable. Recent strategies to target oncogenic KRAS have included both direct inhibition of the KRAS protein and indirect inhibition of its activity by targeting upstream and downstream signaling pathway mediators. A high-throughput screen of multicell-type tumor spheroids was designed to identify active combinations of targeted small molecules and KRAS pathway inhibitors. Inhibitors of the nonreceptor protein tyrosine phosphatase Src homology 2 domain–containing protein tyrosine phosphatase (SHP2) and the guanine nucleotide exchange factor Son of Sevenless homolog 1 (SOS1) were tested to evaluate indirect upstream pathway inhibition, whereas sotorasib directly inhibited the KRAS G12C variant. As single agents, sotorasib and the SHP2 inhibitor batoprotafib (TNO155) exhibited selectivity toward spheroids with KRAS G12C, whereas the SOS1 inhibitor BI-3406 showed varying activity across KRAS variants. Vertical inhibition of the rat sarcoma virus (RAS)/MEK/ERK pathway by targeting SHP2 or SOS1 and the downstream kinases MEK (trametinib) or ERK (temuterkib) was highly effective. Inhibition of upstream tyrosine receptor kinases with nintedanib in combination with batoprotafib or BI-3406 was also effective and, in combination with sotorasib, demonstrated synergy in spheroids harboring KRAS G12C. Dual inhibition of the RAS/MEK/ERK and PI3K/Ak strain transforming (AKT)/mTOR pathways by batoprotafib or sotorasib with either the mTORC1/2 inhibitor sapanisertib or the AKT inhibitor ipatasertib demonstrated combination activity, primarily in spheroids harboring KRAS G12C. The BCL-2 inhibitor venetoclax, in combination with sotorasib, batoprotafib, or BI-3406, resulted in additive and synergistic cytotoxicity. Lastly, concurrent inhibition of the KRAS pathway with sotorasib and batoprotafib demonstrated combination activity in spheroids containing KRAS G12C.

Significance::

KRAS variants are oncogenic drivers for a range of human cancers. Multiple combinations of small-molecule agents that target RAS signaling were screened and reduced the viability of multicell-type tumor spheroids from a variety of human solid tumors. Combinations warranting further testing were identified.

Genentech sinks SHP2 pact, leaving Relay to race thinning field

Genentech tested GDC-1971 in four phase 1 clinical trials. Roche’s Genentech has scuttled its SHP2 inhibitor pact, returning the rights to Relay Therapeutics after plowing more than $120 million into the alliance. The action is the latest in a series of terminations of SHP2 deals. In 2021, Genentech paid $75 million upfront to license Relay’s SHP2 inhibitor, a molecule variously called RLY-1971, migoprotafib or GDC-1971. The outlay reflected the company’s belief in the promise of pairing the molecule with its KRAS G12C inhibitor GDC-6036. Several years and $45 million in milestones later, Genentech has decided to terminate the R&D alliance without cause, depriving Relay of the chance to pocket another $675 million in success-based paydays. The termination of the deal is part of a broader softening of Big Pharma interest in SHP2. Sanofi began the exodus by axing its Revolution Medicines pact in 2022. AbbVie was the next to rethink its interest in SHP2, culling a deal with Jacobio in 2023, and Bristol Myers Squibb extended the trend by terminating its agreement with BridgeBio Pharma earlier this year. Amid that activity, Erasca axed its SHP2 program. Some companies have kept the faith as the wider sector has soured on SHP2. Merck & Co. entered into a SHP2 pact with Taiho and Astex in 2021 and is now enrolling patients in a phase 1 trial related to the target. Novartis is evaluating a SHP2 inhibitor, TNO155, in combination with a KRAS G12C candidate in a phase 1/2 trial. A clutch of smaller companies are still in the race too. Genentech tested GDC-1971 in four phase 1 clinical trials, three of which are currently recruiting. The active trials are assessing the SHP2 inhibitor in patients with advanced solid tumors including colorectal and non-small cell lung cancer. Depending on the setting, Genentech has paired GDC-1971 with Erbitux, Tagrisso, Tecentriq and its KRAS G12C inhibitor GDC-6036.

Analysis on the Clinical Research Progress of SHP2 Inhibitors

From the perspective of therapeutic interventions, proteins involved in tyrosine phosphorylation in signal transduction pathways are considered potential targets in drug development, as these proteins transmit various growth factor signals during the occurrence and development of tumors. Increased expression or mutations of SHP2 can lead to various solid tumors, such as leukemia, breast cancer, colon cancer, and melanoma. Since SHP2 is a regulator of proliferative signaling pathways (e.g., Ras/MAPK, PI3K/Akt), dysregulation of these pathways may lead to tumorigenesis. Research shows that 35% of Juvenile Myelomonocytic Leukemia (JMML) carries PTPN11 mutations, mainly located in the N-SH2 and PTP domains, which may impair the self-inhibition function of SHP2, leading to persistent activation of SHP2 protein. Expression of activated mutant forms of SHP2, such as D61Y and E76K, in hematopoietic cells promotes cell cycle progression and survival of hematopoietic progenitor cells, and results in sustained activation of PI3K/Akt and Ras/MAPK. In addition to JMML, SHP2 mutants have also been found in myeloid tumors, such as Acute Myeloid Leukemia, Chronic Myelomonocytic Leukemia, Myelodysplastic Syndromes, Acute Lymphoblastic Leukemia, primarily among children, and are rarely found in adults. The expression of SHP2 is upregulated to 70% in ductal invasive breast cancer, and the SHP2-binding protein GAB2 is also overexpressed in 10-15% of human breast cancers. Downregulation of the SHP2 gene can significantly inhibit the proliferation of breast tumor initiating cells and block the maintenance and progression of breast cancer. SHP2 levels are also often elevated in melanoma, with higher levels of expression closely related to poorer prognosis and a metastatic phenotype. Moreover, inhibiting SHP2 can suppress the growth of RTK-driven cancer cells, such as EGFR-L858R-driven lung adenocarcinoma, BRAF-mutated colon cancer, and KRAS-mutated breast cancer. In addition, in KRAS-mutated non-small cell lung cancer (NSCLC), inhibiting SHP2 can promote tumor aging, thereby triggering the immune system to eliminate cancer cells. Furthermore, analysis of tumor samples from advanced EGFR mutation-positive NSCLC patients revealed that those with higher SHP2 and ILK expression benefitted less from EGFR-TKI treatment than those with lower levels.
