The primary objective of this study is to assess the effect of a single therapeutic (50 mg) oral dose of omecamtiv mecarbil (OM) on the QT interval / QT interval corrected for heart rate (QTc), relative to placebo, in healthy adults.
The QT interval is the section on an electrocardiogram (ECG) that represents the time it takes for the electrical system to fire an impulse through the ventricles and then recharge, or the time it takes for the heart muscle to contract and then recover.

BACKGROUND:

Older age is associated with a high prevalence of comorbidities and worse cardiovascular (CV) outcomes in patients with heart failure (HF) with reduced ejection fraction. Omecamtiv mecarbil, a selective cardiac myosin activator, exerts minimal effects on blood pressure and heart rate and has limited extracardiac activity, and thus it may be well-tolerated in older individuals. The safety profile and clinical benefits of omecamtiv mecarbil across the age spectrum remain uncertain.

OBJECTIVES:

This study aims to assess CV outcomes, treatment response, and tolerability to omecamtiv mecarbil according to age in patients enrolled in the GALACTIC-HF (Global Approach to Lowering Adverse Cardiac Outcomes Through Improving Contractility in Heart Failure) trial.

METHODS:

GALACTIC-HF was a randomized, global, double-blind clinical trial testing omecamtiv mecarbil vs placebo in patients with symptomatic HF with reduced ejection fraction. Key eligibility criteria included an age between 18-85 years, elevated natriuretic peptides, and a left ventricular ejection fraction (LVEF) ≤35%. We examined the treatment effect of omecamtiv mecarbil vs placebo for the primary endpoint of CV death or first HF event (hospitalization or urgent visit for HF) in both the overall population and the severe HF subgroup (LVEF ≤30%, NYHA functional class III/IV, HF hospitalization within 6 months), as well as safety outcomes according to prespecified age groups (<65 or ≥65 years).

RESULTS:

A total of 8,232 patients (age 64.5 ± 11.4 years, 54.5% ≥65 years, 21% female) were included. The rate of the primary outcome was 21.4 per 100 patient years (py) in those <65 years of age and 28.8 per 100 py in those ≥65 years of age. The treatment effect of omecamtiv mecarbil for the primary outcome (HR: 0.92 [95% CI: 0.86-0.99]; P = 0.03) was consistent in age groups (P_interaction = 0.76) and irrespective of age as a continuous variable (P_interaction = 0.19, after adjusting for treatment interactions with known modifiers of the effects of omecamtiv mecarbil: LVEF and baseline atrial fibrillation status). In patients with severe HF, omecamtiv mecarbil significantly reduced the risk of the primary outcome in both patients <65 years of age (HR: 0.77 [95% CI: 0.64-0.92]) and those ≥65 years of age (HR: 0.83 [95% CI: 0.71-0.97]; P_interaction = 0.47). The safety profile of omecamtiv mecarbil was consistent irrespective of age (P_interaction > 0.05 for all).

CONCLUSIONS:

In GALACTIC-HF, older individuals were well-represented and faced higher risks of CV events. Treatment with omecamtiv mecarbil was safe irrespective of age and reduced the risk of CV death or first HF event across the age spectrum, especially in those with severe HF. Efficacy of cardiac myosin activators compared to placebo in patients with heart failure and reduced ejection fraction: A systematic review and meta-analysis of randomized controlled trials

BACKGROUND:

Cardiac myosin activators (CMAs) have been explored as a potential therapy to enhance cardiac contractility in patients with heart failure with reduced ejection fraction (HFrEF). However, their effect on clinical outcomes, including mortality, cardiovascular (CV) events, and heart failure (HF) symptoms, remains unclear.

OBJECTIVES:

This study aimed to evaluate the efficacy and safety of CMAs compared to placebo in patients with HFrEF through a systematic review and meta-analysis of randomized controlled trials.

METHODS AND RESULTS:

A systematic search of PubMed, Cochrane, Embase, and Scopus identified randomized controlled trials comparing CMAs to placebo in HFrEF patients. Meta-analyses were performed using a random-effects model, including 10,019 patients from nine trials, with 5204 receiving CMAs. CMAs had no significant effect on CV death (risk ratio [RR]: 1.01; 95 % confidence interval [CI]: 0.92-1.10; p = 0.85), HF events (RR: 0.95; 95 % CI: 0.89-1.02; p = 0.15), or all-cause mortality (RR: 1.00; 95 % CI: 0.93-1.07; p = 0.95). However, CMAs significantly reduced the composite outcome of CV death, HF hospitalization, and urgent outpatient visits for HF (hazard ratio [HR]: 0.92; 95 % CI: 0.90-0.93; p < 0.00001) and lowered stroke risk compared to placebo (p = 0.006).

CONCLUSION:

CMAs, particularly omecamtiv mecarbil, did not significantly impact all-cause mortality, cardiovascular death, or HF recurrence in patients with HFrEF. However, modest reductions were observed in composite outcomes and stroke. These findings should be interpreted cautiously, and further trials are needed to validate their clinical role.

BACKGROUND:

Lactate dehydrogenase (LDH) is a cytoplasmic enzyme found in most cells. Increased LDH levels are a nonspecific measure of cellular injury and may be prognostically important in heart failure (HF).

OBJECTIVES:

This study aims to assess the relationship between LDH and clinical characteristics and outcomes in heart failure with reduced ejection fraction (HFrEF).

METHODS:

Using data from GALACTIC-HF, a phase 3, randomized, placebo-controlled trial evaluating the efficacy and safety of omecamtiv mecarbil (OM) in patients with HFrEF, the relationship between LDH and clinical outcomes was analyzed. The incremental value of LDH added to a validated prognostic model (PREDICT-HF) was also calculated using Harrell's C statistic, integrated discrimination index (IDI), and net reclassification index (NRI).

RESULTS:

In GALACTIC-HF, baseline LDH data were available for 8,179 patients, including 6,138 outpatients. Patients with higher LDH were more frequently female and had worse HF status. They were also more likely to have elevated serum creatinine, liver enzymes, creatine kinase, NT-proBNP, and high-sensitivity troponin I. Compared with patients in the lowest LDH (Q1: 155 U/L [25th-75th percentile: 144-163 U/L]), the HRs for the primary outcome (first HF event or cardiovascular death) were Q2: 183 U/L (25th-75th percentile: 177-188 U/L); HR: 1.15 [95% CI: 1.02-1.31 Hazard ratio does not have unit]; Q3: 207 U/L (25th-75th percentile: 201-215 U/L); HR: 1.39 [95% CI: 1.23-1.58]; and Q4: 253 U/L (25th-75th percentile: 236-280 U/L); HR: 1.84 [95% CI: 1.62-2.08], respectively. Even after adjustment, elevated LDH remained independently associated with higher HR. When added to the PREDICT-HF risk model, baseline LDH improved Harrell's C statistic, IDI, and NRI for the primary outcome.

CONCLUSIONS:

Cytokinetics Announces European Commission Approval of MYQORZO® (aficamten) for the Treatment of Adults with Symptomatic Obstructive Hypertrophic Cardiomyopathy

European Commission Approval Based on Results of SEQUOIA-HCM First European Launch Expected in Germany in Q2 2026 SOUTH SAN FRANCISCO, Calif., Feb. 17, 2026 (GLOBE NEWSWIRE) -- Cytokinetics, Incorporated (Nasdaq: CYTK) today announced that the European Commission (EC) has approved MYQORZO® ( aficamten), 5 mg, 10 mg, 15 mg and 20 mg tablets for the treatment of symptomatic (New York Heart Association, NYHA, class II-III) obstructive hypertrophic cardiomyopathy (oHCM) in adult patients. MYQORZO is an allosteric and reversible inhibitor of cardiac myosin motor activity. In patients with oHCM, myosin inhibition with MYQORZO reduces cardiac contractility and consequently, left ventricular outflow tract (LVOT) obstruction. “The approval of MYQORZO in the European Union is an important milestone towards bringing this medicine to more patients living with obstructive HCM around the world,” said Robert I. About SEQUOIA-HCM The EC approval is based on the positive results from the pivotal Phase 3 clinical trial, SEQUOIA-HCM, published in the New England Journal of Medicine , which demonstrated robust efficacy, safety, and clinically meaningful benefits across symptoms, exercise capacity, hemodynamics, and biomarker endpoints. The results from SEQUOIA-HCM showed that treatment with MYQORZO for 24 weeks significantly improved exercise capacity compared to placebo, increasing peak oxygen uptake (pVO 2 ) measured by cardiopulmonary exercise testing (CPET) by 1.76 mL/kg/min compared to baseline in patients treated with MYQORZO versus 0.0 mL/kg/min in patients treated with placebo (least square mean (LSM) difference [95% CI] of 1.74 mL/kg/min [1.04 - 2.44]; p=0.000002). 1 The treatment effect of MYQORZO was consistent across all prespecified subgroups, including age, sex, patient baseline characteristics, and in patients receiving or not receiving background beta-blocker therapy. During the 24-week treatment period, 3.5% of patients in the treatment group experienced a reversible dose related reduction in left ventricular ejection fraction (LVEF) to < 50% (median 47%; range 34% to 49%). One patient in the treatment group experienced an asymptomatic LVEF < 40%. Reductions in LVEF to < 50% did not require treatment interruption and were not associated with clinical heart failure. The most commonly reported adverse reactions observed with MYQORZO are dizziness (4.2%), systolic dysfunction defined as LVEF < 50% (3.5%), palpitations (7%) and hypertension (7.7%). About MYQORZO® ( aficamten ) MYQORZO® ( aficamten ) is a cardiac myosin inhibitor approved in the U.S., China and European Union for the treatment of symptomatic obstructive hypertrophic cardiomyopathy (oHCM). In patients with HCM, myosin inhibition with MYQORZO reduces cardiac contractility and consequently, left ventricular outflow tract (LVOT) obstruction. MYQORZO was engineered to achieve a predictable exposure response, rapid onset of action and reversibility. 2 Aficamten is also under clinical investigation in ACACIA-HCM, a Phase 3 trial in patients with non-obstructive HCM (nHCM) and CEDAR-HCM, in a pediatric population with oHCM. Aficamten has not been deemed safe or effective for use in either of these patient populations. In addition, aficamten is being studied in FOREST-HCM, an open-label extension clinical study. About Hypertrophic Cardiomyopathy Hypertrophic cardiomyopathy (HCM) is a disease in which the heart muscle becomes abnormally thick. HCM can be obstructive, when thickened muscle blocks blood flow, or non-obstructive, when blood flow is not blocked but heart function is still affected. In obstructive HCM, the thickening of cardiac muscle leads to the inside of the left ventricle becoming smaller, stiffer and less able to relax and fill with blood. Ultimately, HCM limits the heart’s pumping function, leading to reduced exercise capacity and a variety of symptoms. HCM is the most common monogenic inherited cardiovascular disorder, affecting approximately 1 out of 500 Europeans, according to the European Society of Cardiology guidelines. 3 Approximately half of patients with HCM have obstructive HCM (oHCM) and half have non-obstructive HCM (nHCM). 4 People with HCM are at high risk of also developing cardiovascular complications including atrial fibrillation, stroke and mitral valve disease. 5 People with HCM are at risk for potentially fatal ventricular arrhythmias and it is one of the leading causes of sudden cardiac death in younger people or athletes. 6 A subset of patients with HCM are at high risk of progressive disease leading to dilated cardiomyopathy and heart failure necessitating cardiac transplantation. Cytokinetics Announces Recipients of Its Eighth Annual Communications Grant Program

Grants Awarded to Two Patient Advocacy Organizations to Support Communications and Community Outreach SOUTH SAN FRANCISCO, Calif., Feb. 09, 2026 (GLOBE NEWSWIRE) -- Cytokinetics, Incorporated (Nasdaq: CYTK) today announced the recipients of its eighth annual Communications Grant Program, intended to support increased communications, awareness building and community engagement. Two grants were awarded this year to patient advocacy organizations serving the hypertrophic cardiomyopathy (HCM) community. The grants will support the organizations in expanding their reach, awareness and community engagement by providing resources for new or crucial communications, marketing activities or outreach initiatives that would otherwise be challenging to implement. “This year’s Cytokinetics Communications Grant Program recipients reflect the power of thoughtful, patient-centered communication in HCM,” said Mary Pomerantz, Cytokinetics’ Senior Director of Patient Advocacy and Engagement. “Through these grants, we are supporting important initiatives designed to expand awareness and education of HCM, amplify patient voices and strengthen community connections to help make a difference for patients and their families.” The recipients of the 2026 Cytokinetics Communications Grants include: Camp Taylor: Camp Taylor offers free, medically supervised programs for children with heart disease, including HCM. With support from the grant, Camp Taylor will produce Living with HCM: Youth Voices from Camp Taylor, a digital communications initiative designed to expand awareness and education around HCM. The project will include a five-part video series and a companion information toolkit for use in pediatric cardiology clinics. The video series will feature youth and families sharing their lived experiences with HCM in plain language, with bilingual subtitles to reach both English- and Spanish-speaking audiences. The clinic toolkit will provide printed and digital resources, including QR code cards that link families directly to a video library, helping healthcare providers connect newly diagnosed families with accurate, reassuring, and patient-centered information. The Canadian Sudden Arrhythmia Death Syndrome (SADS) Foundation: The Canadian Sudden Arrhythmia Death Syndromes (SADS) Foundation is a registered Canadian charity and the only patient advocacy organization in Canada dedicated to supporting individuals and families affected by inherited cardiac disorders. With support from the grant, the Foundation will develop Think. Know.
