Agios Pharmaceuticals is scrapping development of tebapivat in lower-risk myelodysplastic syndromes (LR-MDS) after a Phase IIb study fell short of the bar needed to move the drug forward, though the company still believes its oral pyruvate kinase (PK) activator may have a future in sickle-cell disease (SCD).The trial enrolled 65 heavily pretreated patients with LR-MDS and anaemia, testing three once-daily dose levels of tebapivat over 24 weeks. The primary endpoint measured transfusion independence, defined as eight consecutive weeks without transfusions."While tebapivat demonstrated evidence of biological activity, clinical benefit was not observed in a sufficient proportion of patients or subgroup of patients to meet [our] predefined threshold for advancement in LR-MDS," the company said in a release Friday. Agios noted that its drug was nevertheless well tolerated across all dose levels, with no new safety signals identified.Sarah Gheuens, chief medical officer and head of R&D, said the findings "underscore the biological complexity of lower-risk myelodysplastic syndromes and the challenges of identifying patients most likely to benefit."Still, Agios isn't abandoning the candidate all together. "PK activation remains a clinically validated mechanism, and we continue to see significant potential for tebapivat as a next-generation medicine in sickle-cell disease," Gheuens added. Topline data from a Phase II trial in that indication are due in the second half.In April, Novo Nordisk reported that its oral PK activator etavopivat met both co-primary endpoints in the Phase III HIBISCUS study in SCD, showing reductions in vaso-occlusive crises alongside improvements in haemoglobin response against placebo.
