The FDA is yet to approve a therapy specifically for FSGS. \n Orphan drug specialist Amicus Therapeutics is paying Dimerix $30 million cash for U.S. rights to the phase 3 prospect DMX-200, establishing it as a challenger to Travere Therapeutics in a rare kidney disease. New Jersey-based Amicus’ hunt for de-risked assets that could address a significant unmet medical need led it to Australia. Melbourne-based Dimerix began a phase 3 study of the CCR2 inhibitor DMX-200 in focal segmental glomerulosclerosis (FSGS) kidney disease in 2022. The Australian biotech recently aligned with the FDA on the use of proteinuria as an appropriate primary endpoint for traditional approval. Dimerix reported interim results on the effect of DMX-200 on proteinuria last year. With enrollment well underway and another interim analysis coming up, data that could support filings for FDA approval is in sight.As the program advances, Amicus could pay Dimerix up to $75 million in development and regulatory milestones. A further $35 million is tied to the first sale of the drug. Beyond that, Amicus has committed up to $410 million in sales milestones and $40 million tied to potential future indications.   The FDA is yet to approve a therapy specifically for FSGS. That could change soon. Travere filed for FDA approval of Filspari in the indication this March. Amicus’ willingness to bet on DMX-200 despite the potential of a rival that is near approval reflects a belief that there is room for multiple approaches in FSGS.Jeffrey Castelli, chief development officer at Amicus, said on an earnings call Thursday that DMX-200 “is specifically targeting the inflammatory signaling in the damaged kidney.” Other molecules, in contrast, are focused on hemodynamics. Some patients will respond better to one drug than the other, Castelli said, and physicians will try to figure out which is the right approach for each individual.  “With DMX-200, there are ways you can look at how much the inflammatory component is driving things. Those are going to be the patients we\'re going to see respond best in the trials, and those are going to be the patients that I think doctors will want to focus on,” Castelli said. ChemoCentryx, now part of Amgen, was developing a CCR2 inhibitor with Vifor Fresenius Medical Care Renal Pharma until a phase 2 flop scuttled the FSGS program in 2020. Castelli discussed the differences between DMX-200 and earlier candidates, contrasting attempts to directly inhibit MCP-1 binding to the downstream approach of Dimerix’s molecule. “What happened with traditional binding inhibitors is MCP-1 couldn\'t dock. It actually led to a rebound effect where MCP-1 levels dramatically increase, which then would compete with the inhibitor and kind of lead to a muting effect,” Castelli said. “The beauty of the downstream blocking is that MCP-1 can still dock. That leads to MCP-1 degradation.”Amicus believes the mechanism of DMX-200 will “really maximize the ability to shut down the monocyte inflammation,” Castelli said. The downstream effects will also allow “monocytes to continue their job elsewhere ... so they can still fight infections as they should outside of the kidney.”

The analysis was conducted after 48 weeks of treatment with 10 mg/kg WVE-N531 dosed every two weeks (Q2W). FORWARD-53 achieved all trial goals, demonstrating sustained and industry-leading exon skipping, muscle concentrations and dystrophin restoration through 48 weeks and a 61-day tissue half-life that supports monthly dosing. WVE-N531 continues to be safe and well-tolerated. Additionally, the data demonstrate substantial decreases in inflammation and necrosis, a statistically significant reversal of muscle fibrosis (28.6% reduction between week 24 to 48; p<0.01), and a transition from regeneration to maturation of muscle. A 50% decline (<0.001) in creatine kinase (CK), as well as decreases in IL-6 and MCP-1, were also observed. Time-to-Rise (TTR) data demonstrate a statistically significant and clinically meaningful 3.8-second improvement versus natural history (p<0.05), which is the largest effect observed relative to any approved dystrophin restoration therapy at 48 weeks. Detailed Results from FORWARD-53 Eleven boys amenable to exon 53 skipping (age 5-11; 10 ambulatory and 1 non-ambulatory) are enrolled in the ongoing open-label FORWARD-53 trial. Biopsy data are from eight of the 11 boys (all ambulatory and who had biopsies at 24 and 48 weeks) while safety and functional outcome assessments were available for all participants. All 11 boys have advanced to the extension portion of the study where they are now receiving monthly doses of WVE-N531. Results after 48 weeks of 10 mg/kg dosing every two weeks include: Safety and Tolerability WVE-N531 was safe and well-tolerated through 48 weeks. All treatment-related adverse events were mild to moderate in intensity. There were no Serious Adverse Events and no discontinuations due to any causes. Results from Muscle Biopsies Dystrophin and exon skipping: Dystrophin expression stabilized between 24 and 48 weeks of dosing with a mean of 7.8% [95% CI: 5.4-10.3%; 24-week dystrophin was 9.0% (95% CI: 6.5-11.5%) and 48-week dystrophin was 6.4% (95% CI: 3.8-9.0%); muscle content-adjusted dystrophin as measured by western blot]. The difference between the 24- and 48-week mean dystrophin measurements is within the established 30-35% inter-assay variability of the western blot, and consistency between these timepoints was confirmed with an orthogonal assay. 88% of boys (7/8) achieved greater than 5% average dystrophin between 24 and 48 weeks. Mean exon skipping reached steady state at 6 weeks and was consistent through 48 weeks of dosing with a mean of 54% (95% CI: 46-63%). Reversal of muscle damage: Participants showed significant and unprecedented improvements in multiple indicators of muscle health through 48 weeks, indicating a shift from dystrophic muscle towards healthy muscle. These data include: Decreases of the median muscle necrosis and inflammation scores from 2 to 1(representing mild damage) in muscle pathology. Decreases in markers of inflammation (MCP-1 and IL-6). A statistically significant decline in the amount of muscle fibrosis between 24 and 48 weeks (mean decrease of 28.6%, p<0.01). NSAA showed positive trends favoring WVE-N531 relative to natural history (1.2-point improvement; not significant). Positive trends were also observed among WVE-N531-treated boys (n=11) on grip strength versus baseline. TTR showed a statistically significant and clinically meaningful 3.8-second difference (p<0.05) favoring WVE-N531 compared with natural history. QUEBEC CITY--( BUSINESS WIRE )--Devonian Health Group Inc. (“ Devonian ” or the “ Corporation ”) ( TSXV: GSD; OTCQB: DVHGF ), a clinical stage corporation focused on developing unique solutions to inflammatory diseases, today announced the results of its in vitro comparative study, which demonstrate the anti-inflammatory effect of Thykamine™, with superior potency on the inflammatory biomarkers as compared to currently marketed anti-inflammatory agents, which included various corticosteroids and a phosphodiesterase inhibitor. The assays were conducted at Department of Microbiology, Infectious Diseases, and Immunology in the Faculty of Medicine at Université Laval (Quebec City) under the leadership of Pr Louis Flamand, PhD, MBA (1) in collaboration with Dr John Sampalis, PhD, Epidemiologist, Medicine Faculty, McGill University and Dr Charles Lynde, MD from Lynde Dermatology and Associate Professor in the Department of Medicine at the University of Toronto. The primary objective of the study was to compare the anti-inflammatory potency of Thykamine™ to other anti-inflammatory agents in the market, specifically betamethasone valerate, clobetasol, crisaborole, 21-acetate hydrocortisone, pimecrolimus and prednisone. The experiment involved treating lipopolysaccharide (LPS) stimulated U937 monocytic cells with each agent. Anti-inflammatory potency was assessed by the percent inhibition of the activity of four cytokines induced by each agent. The four cytokines tested were MCP-1, MIP-1α, MIP-1β and RANTES that have been implicated in several autoimmune diseases. Percent inhibition was calculated as the reduction in lipopolysaccharide (LPS) induced cytokine activity after addition of the agent. Given that the aim of anti-inflammatory drugs is to dampen cytokine induced inflammation, these results can be used as inference to overall anti-inflammatory efficacy. All agents were tested at four increasing concentrations except for pimecrolimus that was tested at three concentrations. All culture conditions were run in hexaplicate (n=6). Cell-free supernatants were collected and immediately assayed for cytokine concentration using Pro-Carta 6 plex (Life Technologies). U937 secreted amounts of IL-6 or IL-10 that were below the detection limits of the assay and were not included in the analyses. When compared to the other agents, Thykamine™ showed clinically important and statistically significant higher inhibition potency for MCP-1 (by 28% to 136%; P< 0.009), MIP-Alpha (by 30% to 108%; P < 0.001), MIP-1 Beta (by 39% to 111%; P < 0.001). For RANTES higher inhibition was observed for all agents (by 47% to 258%) except for Crisaborole and Prednisone. In this assay, pimecrolimus did not inhibit any of the cytokine tested. “ These results support the wide spectrum anti-inflammatory properties of Thykamine™ with potency that is superior to currently marketed anti-inflammatory agents.
