Investigating Pre-Procedural Anxiety and Effect of Analgesia on Intrauterine Device Placement and Endometrial Biopsy

The purpose of this study is to evaluate pre-procedural anxiety and patient pain perception with IUD insertion/ EMB procedure when using analgesia versus no analgesia by the end of the study and to evaluate the effect of age, race, parity, type of IUD and anxiety on pain and to offer better pain management in our patient population, undergoing IUD insertion/EMB procedure at the end of our study.

Applying a New Method for Pain Control in Intrauterine Device Insertion

The goal of this clinical trial is to learn if use of topical benzocaine prior to the injection of lidocaine is effective in decreasing pain experienced during IUD insertion in patients 18 or older. The main question it aims to answer is:
Does topical use of benzocaine prior to lidocaine injection during IUD insertion effectively decrease pain experienced?
Researchers will compare benzocaine/lidocaine to benzocaine/placebo, placebo/lidocaine, and placebo/placebo to see if use of benzocaine prior to lidocaine injection works more effectively to decrease pain experienced rather than lidocaine, benzocaine, or placebo use alone.
Participants will:
* Arrive to clinic for previously scheduled IUD insertion
* Be screened, approached by research staff, and consented to join the trial
* Complete a demographic questionnaire
* Be randomly and blindly assigned to one of four groups
* Placebo/Placebo
* Placebo/Lidocaine
* Benzocaine/Placebo
* Benzocaine/Lidocaine
* All groups will be given 600mg of ibuprofen prior to procedure
* Be asked to rate their pain on a visual scale of 1-10 during several distinct points of the procedure.
* Be sent an optional survey to their email after the procedure

Effects of light therapy on amygdala connectivity and serotoninergic system in young adults with subthreshold depression

BACKGROUND:

Bright light therapy (BLT) demonstrates efficacy in alleviating subthreshold depression (StD) among young adults. The amygdala plays a critical role in depression.

METHODS:

StD subjects were divided into BLT group (N=47) and placebo group (N=42). Depression severity was assessed using HAM-D, Centre for Epidemiologic Studies Depression Scale (CES-D) and Beck Depression Inventory (BDI) pre-/post-8-week intervention. Structural/resting-state fMRI scan was conducted. Seed-based static FC (sFC) and dynamic FC (dFC) analyses of the bilateral amygdala and their subfields were conducted.

RESULTS:

Compared to placebo, BLT showed reduced depression scale, and increased sFC of right basolateral amygdala (BLA)/superficial amygdala (SFA)-right middle temporal gyrus (MTG) and dFC of right centralmedial amygdala (CMA) and right inferior orbital frontal gyrus, and decreased sFC of right amygdalostriatal transition/CMA- left thalamus and dFC of right SFA- right medial prefrontal cortex after intervention; while the whole amygdala and its subnuclei volume did not change significantly after BLT. Right BLA-MTG sFC changes positively correlated with BDI improvement. Baseline amygdala sFC/dFC predicted post-BLT symptom changes. BLT-induced right BLA sFC alterations spatially correlated with 5-HT1A/5-HT2A receptor distributions, and right CMA dFC changes with 5-HT1A.

CONCLUSIONS:

Findings suggest BLT modulates amygdala-thalamocortical circuits and serotonergic pathways, highlighting FC biomarkers for treatment efficacy assessment.

Xanthosomes loaded with recombinant TMEM219 for targeting IGFBP-3: A novel approach for targeted drug delivery to ameliorate liver fibrosis

AIMS:

Insulin-like growth factor binding protein-3 (IGFBP-3) activates hepatic stellate cells, enhancing hepatic fibrogenesis. This research aimed to fabricate IGFBP-3 targeted xanthosomes loaded with transmembrane-protein 219 (TMEM219) to target TGFβ/IGFBP-3 signaling pathway in liver fibrosis.

MATERIALS AND METHODS:

Recombinant TMEM219 was expressed, purified, and loaded into xanthosomes using thin-film hydration method. MTT assay performed cytotoxicity on the LX-2 cell line. Liver fibrosis was induced in rats via bile duct ligation, followed by intravenous injection: Sham (SH) and Fibrotic (F) with saline, Short-term (ST) and Long-term (LT) TMEM219 loaded in targeted xanthosomes (0.4 mg/kg), Blank targeted xanthosomes (BST and BLT), and free TMEM219 (Free; 0.4 mg/kg). Evaluations included biochemical parameters, histopathology and immunohistochemistry, α-SMA and COL1A1 mRNA expression, serum IGFBP-3 levels, and TGF-β1 protein expression.

KEY FINDINGS:

TMEM219-loaded xanthosomes exhibited a particle size of 149.9 nm, zeta-potential of -11.4 mV, and loading efficiency of 71.5 %. MTT assay showed lower cytotoxicity of targeted xanthosomes compared to free TMEM219 (p < 0.05). ST and LT groups significantly improved AST and ALT levels, histopathology (reduced necrosis, fibrogenesis, inflammation), α-SMA and COL1A1 expression, serum IGFBP-3, p-AKT and TGF-β1 expression (p < 0.05, p < 0.01).

SIGNIFICANCE:

Improvements in therapeutic parameters indicated reduced liver fibrosis progression. This study demonstrates that TMEM219-xanthosomes effectively target IGFBP-3, significantly reducing liver fibrosis.

Controllable Hydrodebromination of Tribromomethyl Groups via Lewis Base–Borane-Mediated Ionic and Radical Pathways

Abstract:

Compounds bearing mono- and dibromomethyl groups are extensively utilized in synthetic and medicinal chemistry. In this regard, selective debromination of readily or easily accessible tribromomethyl compounds offers a direct and efficient method to access those two moieties. In this work, we introduced Lewis base–borane-mediated selective hydrodebromination of tribromomethyl groups via ionic or radical pathway. Using 4-(dimethylamino)pyridine–borane (DMAP-BH3) as a hydride donor, monohydrodebromination of tribromomethyl groups was achieved via nucleophilic substitution by hydride, delivering dibromomethyl groups. On the other hand, treatment of tribromomethyl compounds with an N-heterocyclic carbene–borane (NHC-BH2CN) as a boryl radical precursor in the presence of dilauroyl peroxide (DLP) as radical initiator afforded monobromomethyl compounds through consecutive bromine atom abstractions to cleave two C–Br bonds. Various tribromomethyl group bearing acetamides and acetates were applied in this developed protocol, showcasing good functional group tolerance and broad substrate scope.

Alliance Pharma agrees to £362m acquisition by DBAY

UK-based Alliance Pharma has agreed to a final increased cash offer from asset management company DBAY Advisors, valuing the former at approximately £362m ($467.5m). The offer, made through a newly formed company called Aegres Bidco Limited, will see DBAY acquire all shares in Alliance, except those it already owns through its affiliates, for 64.75p per share. This represents a 3.6% increase from DBAY’s initial offer of 62.5p per share in January 2025 and a 46% premium on Alliance’s share price as of 9 January 2025, the day before the offer period began. Alliance Pharma specialises in the marketing and distribution of consumer healthcare products and prescription medicines. Its portfolio includes Kelo-Cote, a scar treatment; and Anbesol (benzocaine), an oral pain relief product. DBAY initially approached Alliance last year, following a series of challenges faced by the company, including the delayed publication of its annual results and the resignation of its CEO Peter Butterfield. Having gradually increased its stake in the company over two years, DBAY now holds a 27.9% share of Alliance. In its initial takeover offer, DBAY stated that it believed Alliance had “strong future prospects” but required significant operational changes and increased investment to maximise its growth potential. The asset management company argued that such changes would be more effectively implemented away from the pressures of the public market. In January 2025, Alliance’s board of directors recommended shareholders accept the initial offer of 62.5p per share. However, after receiving feedback from shareholders, the scheduled general meeting last month was adjourned to allow for further discussions. According to a 10 March filing on the London Stock Exchange, DBAY will fund the acquisition through a combination of equity investments from its affiliated funds and external financing, with a small portion of shares in Midco, the parent company of Bidco, being offered to certain lenders. Shareholders are expected to vote on the proposal at a rescheduled meeting on 13 March 2025. If approved, the deal is anticipated to be completed in the first half of 2025, resulting in Alliance Pharma becoming a privately owned company under DBAY’s control. In September 2024, Alliance reported underlying revenue of £84m for the six months to 30 June 2024, reflecting an increase from £81.4m in the same period the previous year. Profit before tax also rose year-on-year, reaching £12.7m compared to £10.3m in 2023.

Govt bans 156 medicines including antibiotics, painkillers, multivitamins: Here is the full list

New Delhi: In a significant move aimed at safeguarding public health, the Indian government has banned 156 fixed-dose combination (FDC) drugs, including a variety of antibiotics, painkillers, multivitamins, and medications for treating fever and hypertension. This decision marks the largest crackdown on FDCs since 2016, when 344 such drugs were prohibited. The Union Ministry of Health and Family Welfare issued a gazette notification on Thursday, officially prohibiting the manufacture, sale, and distribution of the banned FDCs. The ban was based on the recommendations of an expert panel that reviewed a total of 324 FDCs. Impact on Pharmaceutical Industry The banned FDCs include popular combinations such as mefenamic acid and paracetamol injections, commonly used for pain relief and fever, and omeprazole magnesium with dicyclomine HCl, used to treat abdominal pain. The ban is expected to impact major pharmaceutical companies, including Sun Pharmaceuticals, Cipla, Dr. Reddy's Laboratories, Torrent Pharmaceuticals, and Alkem Laboratories. Insight Pharmaceuticals Issues Voluntary Nationwide Recall of Americaine® 20% Benzocaine Topical Anesthetic Spray Due to the Presence of Benzene

TARRYTOWN, N.Y., Dec. 22, 2023 (GLOBE NEWSWIRE) -- Insight Pharmaceuticals, a Prestige Consumer Healthcare Inc. company (“Insight”), is voluntarily recalling one lot of Americaine® 20% Benzocaine Topical Anesthetic Spray to the consumer level. While benzene is not an ingredient in any Americaine® 20% Benzocaine Topical Anesthetic Spray products, a recent review by our manufacturer and their third-party lab found that a sample from one lot of the product showed a low level of benzene came from the propellant that sprays the product out of the can. Importantly, no other lots of Americaine® 20% Benzocaine Topical Anesthetic Spray (either before or after this batch code) are in the scope of this recall and may continue to be used by consumers safely and as intended. Risk Statement: Benzene is classified as a human carcinogen. Exposure to benzene can occur by inhalation, orally, and through the skin and it potentially can result in cancers including leukemia and blood cancer of the bone marrow and blood disorders which can be life threatening. Insight is recalling these products out of an abundance of caution. To date, the Company has not received any serious adverse events related to this recall. Benzene is ubiquitous in the environment. Humans around the world have daily exposures to it indoors and outdoors from multiple sources. Americaine® 20% Benzocaine Topical Anesthetic Spray is packaged in white aerosol cans with Teal caps with Lot codes located on the bottom of the can. Samples of the recalled lot below have been found to contain trace levels of Benzene related to the propellant that sprays the product out of the can. Benzene is not an ingredient in any Americaine products. Product NDC Lot Code Expiration Package Size Americaine® 20% Benzocaine Topical Anesthetic Spray 63736-378-02 1A16420 01/25 2 oz / 57 g The affected Americaine® 20% Benzocaine Topical Anesthetic Spray lot was distributed nationwide in the United States through a limited number of retailers and online. Insight has notified retailers via overnight mail to remove any remaining recalled product from shelves and follow the instructions provided in the Drug Recall Notification. The company will also offer reimbursement for consumers who have purchased Americaine® 20% Benzocaine Topical Anesthetic Spray marked with the lot code in the table above. Consumers can contact Insight Pharmaceuticals via e-mail at medicalaffairs@prestigebrands.com, through its website at , or by phone at (800) 344-7239 on Monday – Friday 8:30-5:30 eastern time to receive a full refund by providing a picture of the bottom of the can of the Americaine® 20% Benzocaine Topical Anesthetic Spray with the affected lot number. Consumers that have product which is being recalled should stop using the product immediately and appropriately discard after taking the picture. Consumers should contact their physician or healthcare provider if they have experienced any problems that may be related to taking or using this anesthetic product. No serious adverse events have been reported to date.
