Quantitative Analysis of Postoperative Analgesia Following Abdominal Surgery Using Repeated Time-to-Event Modelling of Rescue Analgesic Administration

Postoperative pain remains a common challenge after abdominal surgery despite the use of standard multimodal pain management strategies. Poorly controlled pain can delay recovery, limit early mobilisation, reduce patient comfort, and increase the risk of longer-term pain problems.
This study aims to develop a predictive model to better understand and anticipate patients' postoperative pain relief needs after abdominal surgery. The study will observe adults undergoing elective abdominal surgery and collect information on their pain scores, pain medication use, surgical characteristics, and recovery during the first 48 hours after surgery.
Participants will receive standard postoperative pain management according to usual clinical practice. The study will analyse patterns of rescue pain medication use, including when and how often additional pain relief is required after surgery. The collected information will be used to develop a model that can predict factors associated with increased pain medication requirements.
The goal of this research is to improve understanding of individual differences in postoperative pain and provide a foundation for more personalised pain management strategies in the future. The findings may help healthcare providers identify patients who are more likely to require additional pain relief and support the development of safer and more effective approaches to postoperative pain control.

Intraoperative Placement of Vancomycin-impregnated Calcium Sulfate Beads Conjugated With Analgesics to Improve Spine Surgery Outcomes

The goal of this clinical trial is to learn if using calcium sulfate beads that contain vancomycin (an antibiotic) and pain-relieving medicines during spine surgery can improve recovery compared to standard care with intravenous (IV) morphine and local pain medicines.
Researchers want to know if the sustained release formulation of morphine from these calcium sulfate beads affect:
* Postoperative length of stay
* Postoperative pain scores
* Postoperative narcotic analgesic requirements
All participants in this study will already be scheduled to have spine surgery as part of their regular medical care. During surgery, participants will be assigned to receive either intraoperative placement of calcium sulfate beads with vancomycin alone or vancomycin with morphine and bupivacaine. All patients will still have access to standard of care with IV and PO pain regimens. Participants will then be followed after surgery to measure their pain, recovery, and medication use.
Participation is voluntary, and choosing not to join will not affect a person's medical care.

Ultrasound-Guided Posterior Quadratus Lumborum Block With Bupivacaine and Dexmedetomidine Versus Intrathecal Morphine for Postoperative Analgesia After Cesarean Delivery

Cesarean delivery (CD) is one of the most commonly performed surgical procedures worldwide, with millions of women undergoing the procedure annually (1). Despite advances in surgical techniques and perioperative care, postoperative pain management remains a significant clinical challenge (2). Moderate to severe postoperative pain is commonly reported following CD, which can seriously impair daily activities, hinder maternal-infant bonding, delay recovery, and increase the risk of negative psychological outcomes, including postpartum depression and chronic pain syndromes. Effective postoperative analgesia is crucial not only for maternal comfort but also for optimizing recovery outcomes. Adequate pain control facilitates early mobilization, reduces hospital length of stay, promotes breastfeeding initiation, and strengthens the maternal-infant relationship Consequently, clinicians continuously refine postoperative analgesic protocols to enhance both effectiveness and safety while minimizing adverse effects that could compromise maternal and neonatal well-being. The Challenge of Post-Cesarean Analgesia Intrathecal morphine (ITM) has long been considered the "gold standard" for postoperative analgesia following cesarean delivery due to its excellent analgesic efficacy and prolonged duration of action (3). Administered as a single injection during spinal anesthesia, ITM provides effective pain relief for 12-24 hours postoperatively, making it a cornerstone of enhanced recovery after cesarean (ERAC) protocols. However, ITM is associated with a significant side-effect profile that limits its clinical utility. Higher doses, while offering superior pain control, substantially increase the risk of adverse effects including pruritus, postoperative nausea and vomiting (PONV), urinary retention, and potentially life-threatening respiratory depression. Studies have demonstrated that pruritus occurs in up to 91% of patients receiving 200 mcg of ITM, while respiratory depression risk increases significantly with doses exceeding 0.3 mg (6). Although dose reduction can mitigate these effects, the optimal dose that balances analgesia with an acceptable side-effect profile remains controversial. Some evidence suggests that doses as low as 0.025-0.075 mg may be sufficient when combined with multimodal analgesia (5,6). ltrasound-guided regional nerve blocks have emerged as valuable components of multimodal analgesia, offering the potential for effective pain relief while reducing systemic opioid exposure. Among these techniques, the quadratus lumborum block (QLB) has gained significant attention for abdominal surgeries, including cesarean delivery. The QLB targets the fascial planes surrounding the quadratus lumborum muscle. Unlike the transversus abdominis plane (TAP) block, which primarily provides somatic analgesia of the anterolateral abdominal wall, the QLB offers potential advantages including more extensive sensory blockade (T6-L3) and the ability to address both somatic and visceral pain components through spread of local anesthetic to the paravertebral space (4,7) between groups. Evidence suggests that bilateral QLB provides more effective postoperative pain relief than TAP blocks for cesarean delivery, with better visceral pain control and greater opioid-sparing effects. The addition of dexmedetomidine as an adjuvant to local anesthetics in QLB has shown promising results. Dexmedetomidine, a selective alpha-2 adrenergic agonist, possesses analgesic and sedative properties without respiratory depression. Studies have demonstrated that perineural dexmedetomidine prolongs the duration of local anesthetic action, likely through localized vasoconstriction and inhibition of nerve fiber action potentials. Understanding morphine influence on gut health and depression: Evidence from a rat model

BACKGROUND:

Opioid addiction severely impacts mental health, and recent evidence links opioid use to gut microbiota changes and mood disorders. This study examines how morphine influences gut microbiota, inflammatory responses, and depressive behaviors in rats.

METHODS:

Male Wistar rats received escalating morphine sulfate doses (30 mg/kg) or served as controls. Depressive-like behaviors were assessed via the Forced Swim Test (FST) and Sucrose Preference Test (SPT). Gut microbiota composition was analyzed using qPCR, and pro-inflammatory markers were measured in intestinal tissue.

RESULTS:

Morphine treatment induced depressive-like behaviors, as evidenced by increased immobility time in the FST (120 s vs. 69.5 s in controls; P<0.01) and reduced sucrose preference (40%vs. 70% in controls; P<0.05). Gut microbiota analysis revealed a significant decrease in beneficial bacteria, including Bifidobacterium longum (P<0.05) and Lactobacillus rhamnosus (P=0.02), and an increase in pathogenic bacteria such as Fusobacterium nucleatum (P<0.001) and Clostridium difficile (P<0.01). Enterococcus faecalis was also significantly increased in the morphine-treated group (P<0.001). Ileal inflammatory marker analysis showed significantly elevated TNF-α (2.15±0.45, P<0.001) and IL-1β (3.81±1.16, P<0.001) mRNA levels, with a trend towards elevated IL-6 (1.72±1.75; P=0.22).

CONCLUSION:

Morphine treatment significantly alters gut microbiota composition, worsening depressive-like behaviors and inducing intestinal inflammation. These findings highlight the complex link between opioid use, gut health, and mental well-being, suggesting microbiota-targeted therapies may help manage opioid addiction and related psychological disorders. Further research is needed to explore how modulating gut microbiota can lessen the negative effects of opioid therapy.

Targeted Morphine Administration Training to Enhance Nurses’ Knowledge and Self-Efficacy for End of Life Dyspnea

Dyspnea is a prevalent and distressing symptom at the end of life (EOL), affecting up to 90% of dying patients and causing significant suffering for patients and families. Morphine sulfate remains the standard pharmacological treatment for refractory EOL dyspnea, effectively modulating central respiratory drive and alleviating breathlessness perception, distinct from its use in chronic stable breathlessness, where recent evidence shows limited benefit and potential harms. Many nurses, however, lack adequate knowledge and confidence in administering morphine appropriately due to misconceptions about addiction, respiratory depression, and hastening death. This quasi-experimental study evaluated a brief online training module on morphine administration for EOL dyspnea among registered nurses. Of 69 enrollees, 34 provided complete pre- and post-intervention data. Paired t tests revealed significant improvements in knowledge and self-efficacy. These findings highlight the value of targeted, scalable education in addressing knowledge gaps, enhancing nurse confidence, and supporting evidence-based EOL symptom management.

Potential drugs for reducing the occurrence of immune checkpoint inhibitor-induced interstitial lung disease: an exploratory study using the JADER and FAERS databases

BACKGROUND:

Immune checkpoint inhibitors (ICIs) play a central role in cancer immunotherapy. However, the occurrence of immune-related adverse events, especially ICI-induced interstitial lung disease (ICI-ILD), is life-threatening and affects the effectiveness of ICI treatment. This study aimed to explore potential drugs to mitigate ICI-ILD occurrence using data from the Japanese Adverse Drug Event Report (JADER) and the US Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS [JAPIC AERS]).

RESEARCH DESIGN AND METHODS:

We investigated concomitant drugs that reduce ILD associated with four ICIs - nivolumab, pembrolizumab, atezolizumab, and durvalumab - across the JADER and FAERS databases. Subsequently, the identified common concomitant drugs that reduce the occurrence of ICI-ILD were detected and analyzed.

RESULTS:

We found omega-3 fatty acids, loperamide, and amlodipine as common concomitant drugs that reduced ICI-ILD occurrence in both the JADER and FAERS databases. Omega-3 fatty acids reportedly have many effects in animal models of drug-induced ILD, including their association with ILD in humans and anti-inflammatory effects against ICI-ILD. However, loperamide and amlodipine reportedly have minimal effects against ILD, thereby necessitating further evaluation.

CONCLUSION:

Omega-3 fatty acids have emerged as potential agents for reducing ICI-ILD occurrence, as evidenced by findings from two different pharmacovigilance databases.

Fresenius unit dinged in FDA warning letter flagging 'complaint trend' from customers

Following a string of customer complaints, a Form 483 and a recall this past spring, a Fresenius manufacturing unit in Utah is contending with a warning letter from the FDA. The FDA reprimand, dated Aug. 25 and posted to the agency’s website this week, follows an inspection of Fresenius Medical Care’s plant in Ogden, Utah, in early March of this year. In two observations, the FDA’s concerns focused on the company’s failure to thoroughly investigate customer complaints about its products, plus lackluster inspection protocols at the plant to suss out potential particulate contamination. Last August, the Fresenius unit kicked off its own investigation due to a “complaint trend” around “leaking bags” of a certain dialysis solution product it sells, per the FDA write-up, which noted that Fresenius Medical Care eventually received 35 complaints related to some 156 bags from multiple batches. At the time, the company attributed the issue to holes caused during the bag printing process, per the FDA. The manufacturing defect posed the risk of peritonitis, or infection causing the tissue inside the abdomen to become inflamed. But the company “instead assigned the lowest severity level” to the issue, the regulator wrote. The company did launch a related recall this past April, but the FDA seemed to chide Fresenius Medical Care because it “chose not to conduct a recall following the results of your August 2025 investigation.” “Your decision not to recall these batches at that time exposed peritoneal dialysis (PD) patients to potentially non-sterile drug products,” the regulator continued. The FDA believes the Fresenius unit has more work to do yet in assuring it detects leaking units during manufacturing and is able to stop their release and distribution, which the FDA called “especially concerning considering the numerous complaints, Field Alert Reports, and recalled batches in the last three years” at the facility. In a second observation, the FDA also reprimanded Fresenius Medical Care over the Ogden facility’s visual inspection program, which the agency deemed inadequate to ensure sterile drugs were “essentially free” of visible particulates. The issue came down to the company’s failure to “appropriately qualify personnel who performed visual inspections of injectable products,” the FDA explained. In particular, the qualification kits used by the company “did not include adequately representative visible particles” across characteristics like size, and the company’s qualification records weren’t detailed enough for investigators to determine the qualifications of staff who performed visual inspections, the regulator added. Given the nature of the observations, the FDA is recommending that Fresenius Medical Care enlist a current good manufacturing practices (cGMP) consultant to help right the ship at the Ogden plant. Sister company Fresenius Kabi has also grappled with its share of manufacturing pitfalls this summer, recently launching separate recalls of an Actemra biosimilar over the potential presence of glass particles and a batch of morphine sulfate, which had been mislabeled and could instead contain the separate opioid Dilaudid.

Scientists discover a brain “brake” that can shut down chronic pain

Researchers discovered a brain-based “brake” that can shut down chronic nerve pain in mice by calming an overactive pain circuit. Targeting this system more precisely could one day lead to powerful pain relief without affecting opioid receptors throughout the entire body.Deep inside the brain, a small group of nerve cells helps control how strongly pain signals are felt. Under normal conditions, this system can suppress pain traveling through the spinal cord. After nerve damage, however, the same circuitry can become overactive and help sustain chronic pain. Researchers at Washington University School of Medicine in St. Louis have now identified a mechanism that helps explain this switch and may offer a way to reverse it. In mice, they found that receptors on cells in the brain's main alert and stress center can act as biological brakes that restrain pain. These receptors were already known for their role in stress, but the new findings suggest they can also quiet a pain-producing circuit and reduce chronic neuropathic pain caused by nerve injury. The study, published Aug. 17 in Current Biology, points to the locus coeruleus as a possible target for future pain therapies designed to act more precisely within the brain. "Millions of adults live with chronic neuropathic pain caused by nerve damage," said Jordan McCall, PhD, an associate professor in the Center for Clinical Pharmacology in the WashU Medicine Department of Anesthesiology and the study's senior author. "The pain is difficult to treat, and traditional opioid medications bind to receptors throughout the entire body and brain, often leading to side effects, tolerance and addiction risk. Understanding how localized receptors in the locus coeruleus act as gatekeepers could lead to more targeted, effective pain therapies with fewer risks." How Nerve Damage Can Turn Up Pain Neuropathic pain develops when injured nerve fibers repeatedly send abnormal signals to the brain. Those faulty messages can produce shooting, stabbing or burning sensations. Diabetes, viral infections and nerve compression are among the conditions that can lead to this type of pain. To investigate how the process might be stopped, McCall's team, including co-first authors Chao-Cheng Kuo, PhD, a postdoctoral research associate, and Makenzie R. Norris, a former graduate student, focused on the locus coeruleus, a brain region already known to help regulate pain. The researchers first confirmed that nerve injury can transform the locus coeruleus into an active source of pain. When they temporarily silenced cells in this region in mice, animals modeling neuropathic pain became less sensitive to touch and heat compared with healthy mice. Opioid Receptors Act as a Biological Brake The team then examined opioid responsive receptors on cells in the locus coeruleus, focusing especially on mu opioid receptors. Mu opioid receptors are found throughout the brain and spinal cord. When naturally produced opioids in the body, or synthetic opioids such as morphine and fentanyl, bind to these receptors, pain signaling across the nervous system is reduced. Because the locus coeruleus contains many of these receptors, the researchers investigated whether they play a particularly important role in controlling pain there. They removed mu opioid receptors specifically from locus coeruleus brain cells in mice with neuropathic pain. Without those receptors, the animals became even more sensitive to touch and heat than mice whose locus coeruleus cells still had mu opioid receptors. When the researchers restored the receptors to the same neurons, that increased sensitivity was reversed, effectively switching off the heightened pain response. A More Precise Target for Chronic Pain The findings suggest that chronic pain may interfere with the ability of mu opioid receptors to restrain activity in locus coeruleus brain cells. The researchers are now investigating ways to alter activity in the locus coeruleus without affecting opioid receptors throughout the rest of the nervous system. Their goal is to develop therapies that engage mu opioid receptors specifically within this brain region, potentially delivering strong relief from chronic neuropathic pain while reducing the risks associated with drugs that act more broadly across the brain and body.
