Efficacy and Safety of Culmerciclib Plus Anlotinib and Endocrine Therapy for HR-Positive/HER2-Negative Metastatic Breast Cancer With Brain Metastases：a Multicenter, Single-arm, Phase Ⅱ Trial

This study aims to evaluate the efficacy and safety of adding the anti-angiogenic agent anlotinib to the combination of the CDK2/4/6 inhibitor Culmerciclib and endocrine therapy in patients with HR+/HER2- breast cancer and brain metastases. Eligible patients present with newly diagnosed brain metastases or brain metastases that progressed following prior local therapy. It is hoped that this therapeutic combination can achieve disease control in these patients.

Anlotinib plus envafolimab as neoadjuvant therapy in a patient with chemotherapy-refractory advanced endometrial cancer: A case report

We report a case of a 51-y-old female presenting with synchronous double primary malignancies: advanced endometrial cancer and early-stage lung cancer. One year a post-chemotherapy withdrawal due to toxicity, she developed severe urinary retention and poor performance status caused by pelvic tumor progression. Considered ineligible for standard chemoradiotherapy, she received anlotinib (a novel multi-target TKI) combined with envafolimab (a subcutaneous PD-L1 antibody). This regimen resulted in rapid symptom resolution and significant tumor regression, successfully converting the patient to surgical candidacy. She subsequently underwent laparoscopic hysterectomy, bilateral salpingo-oophorectomy, and lymphadenectomy, achieving an R0 resection. The lung lesion was treated with stereotactic body radiation therapy. She remains disease-free at follow-up. The combination of anlotinib and envafolimab shows promise as an effective conversion therapy for endometrial cancer, particularly in patients with advanced stage, poor performance status, or intolerance to intensive treatments.

The Efficacy and Safety of AI With Anlotinib Versus AI for Treating Synovial Sarcoma Patients With Lung Metastasis: A Retrospective Cohort Study

ABSTRACT:

Synovial sarcoma (SS) is a rare and aggressive soft tissue sarcoma frequently associated with lung metastases. While anthracycline plus ifosfamide (AI) remains a standard first‐line regimen, its benefit in SS patients with lung metastases is unknown. Anlotinib, a multi‐target tyrosine kinase inhibitor (TKI), has shown promising effects in sarcoma treatment, but its combination with AI has not been well studied in this setting. This retrospective, single‐center study included 108 SS patients with lung metastases treated between 2007 and 2024, receiving either AI alone ( n = 59) or AI combined with anlotinib ( n = 49). Survival outcomes were assessed using Kaplan–Meier analysis, and adverse events (AEs) were reviewed from clinical records. The results indicated that the combination therapy demonstrated statistically significant improvements in clinical outcomes compared to AI monotherapy, with a median PFS of 8.1 months versus 6.2 months ( p = 0.0250), and a median OS of 14.8 months versus 6.8 months ( p = 0.0033). Most AEs were Grades 1–2 according to common terminology criteria for adverse events (CTCAE) criteria, with manageable toxicity profiles. The combination group exhibited modest but clinically insignificant elevations in hypertension and proteinuria, with no reported treatment‐related mortality. In conclusion, the anlotinib and AI chemotherapy combination regimen shows promising efficacy in prolonging survival duration for SS patients with lung metastases while maintaining an acceptable safety profile. These findings suggest that this combination therapy could be considered as a viable first‐line treatment option for this patient population, warranting further validation through phase III randomized controlled trials.

Heterogeneous inorganic nanomedicine delivery system loaded with anlotinib for enhanced treatment of non-small cell lung cancer (NSCLC)

Non-small cell lung cancer (NSCLC) poses a formidable therapeutic challenge due to drug resistance and limited treatment efficacy. To address this, we developed a novel nanodrug delivery system, SPIO@SiO₂-ANB, which integrates superparamagnetic iron oxide nanoparticles (SPIOs), a silica (SiO₂) shell, and the surface-loaded multi-target tyrosine kinase inhibitor anlotinib (ANB). This platform enhances drug stability, enables tumor-targeted delivery, and induces ferroptosis. The synthesized nanoparticles demonstrated a monodisperse spherical morphology, pH-responsive ANB release, and efficient cellular internalization. In vitro, SPIO@SiO₂-ANB exhibited superior cytotoxicity against A549 and H460 NSCLC cells compared to free ANB, concurrently elevating intracellular reactive oxygen species (ROS) and modulating key ferroptosis-related proteins (COX-2, xCT, GPX4). In vivo studies using xenograft models revealed improved tumor accumulation and retention of SPIO@SiO₂-ANB, leading to significant tumor growth inhibition, enhanced apoptosis, and suppressed angiogenesis. This work presents a synergistic therapeutic strategy that combines ferroptosis induction with targeted therapy, offering a promising avenue for the safe and effective treatment of NSCLC.

Akeso Presents Updated Data on Cadonilimab Combination Therapy in PD-(L)1 Inhibitor-Resistant Advanced NSCLC at ELCC 2026

HONG KONG, April 6, 2026 /PRNewswire/ -- Akeso, Inc. (9926.HK) ("Akeso" or the "Company") announced that, at the 2026 European Lung Cancer Congress (ELCC 2026), it reported updated results with a median follow-up of 21.45 months from a prospective, open-label, single-arm, multicenter Phase Ib/II study evaluating cadonilimab in combination with anlotinib and docetaxel in patients with locally advanced or metastatic non-small cell lung cancer (NSCLC) who progressed after prior PD-(L)1 inhibitor-based therapy. Disease Control and Response Durability: The Disease Control Rate (DCR) reached 95.2%; the Objective Response Rate (ORR) was 26.2%, and the median Duration of Response (DoR) was 6.0 months. Patients who achieved circulating tumor DNA (ctDNA) clearance had a median PFS of 9.1 months. After the first treatment cycle (C1), the ctDNA detection rate decreased from 1.5% to 0.5%, demonstrating the regimen's depth of anti-tumor activity at the molecular level. Lancet: Chia Tai’s benmelstobart and anlotinib combo outperforms pembrolizumab in lung cancer trial

Researchers across 79 centers in China reported that a combination regimen of programmed death-ligand 1 (PD-L1) inhibitor benmelstobart plus the multi-kinase inhibitor anlotinib produced statistically significant improvements in progression-free survival over pembrolizumab monotherapy in PD-L1-positive, driver gene-negative advanced non-small-cell lung cancer (NSCLC), according to the CAMPASS Phase III trial. The results were published in The Lancet Oncology. The trial, funded by Chia Tai Tianqing Pharmaceutical Group, enrolled 531 patients and represents one of the first randomized Phase III datasets to demonstrate head-to-head superiority over pembrolizumab in this setting. However, a substantial proportion of patients progress within a year, and response rates in the broader PD-L1-positive population—defined as tumor proportion score (TPS) of 1% or greater—remain around 30% to 40%. The CAMPASS trial was designed to test whether adding anlotinib, a multi-target anti-angiogenic tyrosine kinase inhibitor, to the PD-L1-targeted benmelstobart could improve outcomes without the addition of chemotherapy. The CAMPASS results The trial randomized patients 2:1, with 354 receiving benmelstobart plus anlotinib and 177 receiving pembrolizumab plus placebo. Patients were aged 18 to 75 years, had an ECOG performance status of 0 or 1, and had no targetable EGFR or ALK mutations. Both squamous and non-squamous histologies were included. Enrollment ran from August 2021 through December 2022 across 79 sites in China. The primary endpoint was progression-free survival assessed by blinded independent central review per RECIST v1.1. Median PFS in the experimental arm was not reached at the time of analysis, compared with 8.9 months in the pembrolizumab arm. The hazard ratio was 0.51 (95% CI: 0.39–0.67; p<0.0001). The 12-month PFS rate was approximately 60% with benmelstobart plus anlotinib versus approximately 40% with pembrolizumab. The PFS benefit was consistent across prespecified subgroups, including patients with TPS 1%–49% and ≥50%, as well as across squamous and non-squamous histologies. Overall survival data, while still maturing with both medians not yet reached, showed a hazard ratio of 0.62 (95% CI: 0.44–0.88; p0.007), indicating an early OS signal in favor of the combination. The objective response rate was approximately 63% in the experimental arm versus 39% in the pembrolizumab arm, a statistically significant difference. The mechanistic rationale for the combination centers on the relationship between tumor angiogenesis and immune suppression. Tumors upregulate VEGF signaling to promote vascular growth, but this process also creates an immunosuppressive microenvironment by reducing T-cell infiltration and promoting regulatory T-cell accumulation. Anlotinib inhibits VEGFR, FGFR, PDGFR, and c-Kit, which normalizes tumor vasculature and is thought to improve T-cell trafficking into the tumor. Benmelstobart then blocks the PD-1/PD-L1 axis, reactivating cytotoxic T-cells within a more accessible microenvironment. The hypothesis is that anti-angiogenic therapy potentiates checkpoint inhibition by making tumors more immunologically permissive, rather than relying on checkpoint blockade alone. The safety profile reflected the dual-agent mechanism. Grade 3 or higher treatment-related adverse events occurred in approximately 60% of patients in the benmelstobart plus anlotinib arm, compared with approximately 35% in the pembrolizumab arm. Anlotinib-related toxicities, particularly hypertension and proteinuria, accounted for a meaningful share of the higher adverse event burden. Treatment discontinuation rates were higher in the experimental arm, however, no new or unexpected safety signals were identified. The investigators characterized the toxicity as manageable with dose modifications. Research context The CAMPASS regimen represents one among several competing approaches to the same problem. Akeso’s ivonescimab, a bispecific antibody targeting both PD-1 and VEGF within a single molecule, reported superiority over pembrolizumab in the HARMONi-2 Phase III trial in 2024 in a Chinese PD-L1-positive NSCLC population. Summit Therapeutics holds ex-China rights to ivonescimab, and the asset has attracted considerable attention as a potential pembrolizumab alternative in the first-line setting. The bispecific format is mechanistically adjacent to the CAMPASS combination but consolidates both activities into one molecule, which may carry pharmacokinetic and dosing advantages. Other China-developed IO combinations—including camrelizumab plus apatinib from Jiangsu Hengrui and sintilimab plus anlotinib from Innovent—are also in late-stage evaluation in overlapping populations. Roche’s atezolizumab plus bevacizumab plus chemotherapy (IMpower150) is approved in non-squamous NSCLC but includes cytotoxic chemotherapy, a distinction that the CAMPASS regimen avoids. The chemotherapy-free design is a feature it shares with the ivonescimab approach, with both positioning against pembrolizumab monotherapy in PD-L1-positive advanced NSCLC. The CAMPASS dataset was generated entirely in China, which carries implications for regulatory and commercial strategy. The trial population reflects a Chinese clinical context, and generalizability to Western populations will require additional data or bridging studies before regulatory submissions outside China. The benmelstobart program is also being evaluated in extensive-stage small-cell lung cancer, where a Phase III trial published in Nature Medicine in 2024 suggests Chia Tai Tianqing is pursuing a broader oncology footprint for the combination platform.
