Evaluation of Persistence of Immunogenicity Following an Open-labelled, Randomized Controlled Trial Evaluating Non-inferiority of 1+1 Compared to 2+1 Dosing Schedules of 10-valent and 13-valent Pneumococcal Conjugate Vaccine in South Africa

This study will evaluate the persistence of immunogenicity following a reduced dosing schedule of 10- or 13-valent Pneumococcal Conjugate Vaccine (PCV10, PCV13). This is the follow-up of a randomized controlled trial in which children received a single priming dose of PCV10 or PCV13 (at 6 or 14 weeks of age) followed by booster dose at 9 months of age (1+1 schedule), compared to a 2+1 PCV schedule (6, 14 weeks of age and 9 months of age).

The Effect of Fractional Doses of Pneumococcal Conjugate Vaccines (PCV10 and PCV13) on Immunogenicity and Vaccine-serotype Carriage in Kenyan Infants

Before the introduction of pneumonia vaccines in 2000, between 700,000 - 1 million children died each year as a result of infection with the bacteria Streptococcus pneumoniae and the resulting diseases, namely, meningitis, sepsis and pneumonia. Most of the deaths were in Africa and Asia. Where the vaccines have been introduced, they have been highly effective and have already reduced disease. However, at 10 USD per child, they are not affordable to most low-income countries without financial support from Gavi, the Vaccine Alliance.
This project aims to assess whether lower doses of the two commercially available pneumonia vaccines can protect Kenyan infants as well as the full dose. The results could be used to increase the affordability of the pneumonia vaccine, and enable delivery of the vaccine to continue in the absence of Gavi support.

Immunogenicity and Safety Study of an Investigational Quadrivalent Meningococcal Conjugate Vaccine When Administered Concomitantly With Routine Pediatric Vaccines in Healthy Infants and Toddlers in Europe

Primary objective:
This study aimed to demonstrate the non-inferiority of the antibody response against meningococcal serogroups A, C, Y, and W following the administration of a 3-dose series of MenACYW conjugate vaccine compared to a 3-dose series of a licensed meningococcal vaccine when each vaccine was given concomitantly with routine pediatric vaccines (10-valent pneumococcal vaccine and diphtheria, tetanus, pertussis, hepatitis B, poliomyelitis and Haemophilus influenzae type b [DTaP-IPV-HB-Hib vaccine]) to infants and toddlers 6 weeks to 18 months old

Invasive pneumococcal disease (IPD) caused by Streptococcus pneumoniae remains a significant cause of pediatric morbidity. This study updates the serotype distribution and antibiotic resistance of pediatric IPD in Türkiye for 2019-2025, about a decade after 13-valent pneumococcal conjugate vaccine (PCV13) was introduced in the National Immunization Program. We conducted a multicenter, hospital-based prospective surveillance between January 2019-April 2025 in children <18 y across 28 tertiary hospitals. S. pneumoniae isolates from sterile sites (blood, cerebrospinal fluid, pleural fluid) were confirmed by standard methods and serotyped using the Quellung reaction. Antimicrobial susceptibility was determined by gradient test as per CLSI criteria. A total of 203 samples were identified from 203 pediatric cases (median age 4 y, 56.2% boys). The clinical presentations were bacteremia/sepsis (57.6%) and meningitis (26.1%). The leading serotypes were 3 (14.3%), 14 (10.3%), 19F (7.9%), 19A (6.4%), 9N (4.4%), 15A (4.4%). PCV13 serotypes accounted for 46.8% isolates, while PCV7 and PCV10 serotypes were detected in 23.2% and 24.6%, respectively. Broader vaccines would increase coverage: PCV15 (46.8%) and PCV20 (56.7%). Antibiotic susceptibility (non-meningitis breakpoint) remained high for beta-lactams; 91.9% of isolates were penicillin-susceptible, with only 0.7% fully resistant, and cefotaxime resistance was rare (2.7%). Conversely, macrolide resistance was high, with 60.2% of isolates resistant to erythromycin. A decade after PCV13 implementation, vaccine serotypes - particularly 3, 14, 19F, and 19A - still cause nearly half of pediatric IPD, though non-vaccine serotypes such as 9N and 15A are rising. Higher-valent PCVs may improve protection, and continued surveillance is critical to guide vaccine policy and treatment strategies.

Background . Serotype 8 has emerged as a common non-vaccine serotype causing invasive pneumococcal disease (IPD) in children and adults globally after the introduction of pneumococcal conjugate vaccines (PCV). We aimed to determine serotype 8 incidence and genomic epidemiology in South Africa. Methods . From national, laboratory-based surveillance for IPD in South Africa, we calculated the incidence of serotype 8 and global pneumococcal sequence cluster 3 (GPSC3) before and after PCV introduction. We performed phylodynamic analysis of genomes from clonal complex (CC) 53 and described South African isolates in the context of a global collection of serotype 8 and GPSC3 genomes. Results. Compared to the pre-PCV period, serotype 8 incidence increased in the late-PCV13 period among children <5 years [incidence rate ratio (IRR) 2.3, 95% confidence interval (CI) 1.3–4.1] and adults >64 years (2.8, 1.1–8.3). Similarly, GPSC3 rates increased among children <5 years (IRR 2.4, 95% CI 1.4–4.3) and individuals of all ages (1.7, 1.3–2.1). Serotype 8 CC53 population dynamics predicted at least five clonal expansions since the most recent common ancestor in 1929 (credible interval 1919–1937), with future predictions showing increases in the effective population size despite remaining largely susceptible to first-line treatment. Serotype 8 CC53 isolates from South Africa had a clonal structure when contextualized within a global collection of serotype 8 and GPSC3 genomes. Conclusion. Serotype 8 invasive disease incidence increased in South Africa after PCV introduction. Predictions showed sustained increases over time. Since South Africa switched to a lower-valency PCV10 (Pneumosil, Serum Institute of India) that does not include serotype 8 in 2024, continued surveillance will be crucial to inform PCV policy-making.

BACKGROUND AND AIM:

This study aimed to assess the prevalence, serotype distribution, and antimicrobial susceptibility of Streptococcus pneumoniae (S. pneumoniae) in children aged 6-59 months in Iran, prior to introducing pneumococcal conjugate vaccine (PCV).

METHODS:

Nasopharyngeal (NP) samples were collected from unvaccinated children aged 6-59 months in this multicenter study. S. pneumoniae was identified using culture methods along with biochemical characteristics and confirmed by real-time PCR. Antibiotic resistance testing was performed utilizing the MIC method. Serotyping was accomplished through the utilization of the Quellung test.

RESULTS:

Of the 1063 nasopharynx isolates, 20.4% carried pneumococcus in NP. The five most common isolated serotypes were 23 F, 19 F, 14, 15B and 6 A. High frequency of 15B, as non-vaccine serotypes (NVTs), was observed. Approximately 20% of the isolates were non-susceptible to penicillin, while low resistance rates were recorded against ceftriaxone (8.7%). Overall, 25.4% of pneumococcal strains exhibited multidrug resistance (MDR). The serotype coverage of PCV7, PCV10, and PCV13 were determined as 42.5%, 54.7%, and 61.8%, respectively.

CONCLUSIONS:

S. pneumoniae was detected in 20.4% Iranian children's NP samples, with the serotype 23 F dominancy. Emerging of 15B and lower PCVs prediction coverage compared to other unvaccinated countries, might stem from limited administration of PCV13, environmental/epidemiological factors, and COVID-19-related shifts in colonization. Ongoing surveillance of pneumococcal carriage, serotype distribution, and antimicrobial resistance are required due to the existence of antibiotic resistance and MDR isolates.

During his tenure, he led GSK’s Pharmaceutical R&D restructuring to improve its focus on innovation and productivity and was directly involved in the development of 14 new vaccines, including Shingrix, Cervarix, Mosquirix, Rotarix®, and Synflorix. Moncef holds a PhD in Molecular Biology and Immunology from the Université Libre de Bruxelles and completed his postdoctoral studies at Harvard Medical School and Tufts University.
